(available at www.cbp.org and www.healthpolicy.ucla.edu) recommends that health care reform proposals - such as those proposed by the Governor and Democratic legislative leaders - ensure that families can realistically afford premiums and out-of-pocket costs, such as copayments and deductibles. [...] The report recommends that proposals fully subsidize health care coverage for those who earn up to 200 percent of the poverty line ($41,300 for a family of four) because the cost of housing, food, and other necessities leaves these families with few or no resources to contribute toward health care costs. [...] The report also determines that families need incomes near 300 percent of the poverty line ($61,950 for a family of four) just to afford typical health care costs. [...] "In the debate over health care reform, we can't lose sight of what California families realistically need and can afford," said David Carroll, research director with the California Budget Project, a Sacramento- based nonpartisan public policy research group, and joint author of the report. [...] "Families should not have to choose between paying their rent and taking a sick child to the doctor." "People should be able to afford to use, not simply buy, health care coverage," said Dylan Roby, a joint author of the report and research scientist with the UCLA Center for Health Policy Research, which is based in the UCLA School of Public Health and affiliated with the UCLA School of Public Aff.

Book excerpt: Almost 4.7 million nonelderly adults and children of the seven million Californians who were uninsured for all or part of 2009 will be eligible for insurance as a result of last year's health care reform legislation, according to new data from the 2009 California Health Interview Survey (CHIS 2009). Eligible Californians will obtain coverage either through Medi-Cal or through subsidies to purchase private health insurance in the new California Health Benefit Exchange (CHBE) starting in 2014. The CHBE will also be open to 1.2 million uninsured persons who do not qualify for subsidized premiums due to their income exceeding eligibility levels, but who will benefit from the new marketplace created through the Patient Protection and Affordable Care Act (PPACA). Just over one million uninsured persons do not qualify to participate in either the CHBE or in the Medi-Cal expansion due to their citizenship status. With seven million uninsured residents of California in 2009, the new insurance options made available by the PPACA could face challenges in enrolling these uninsured individuals.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Book excerpt: Diabetes -- a chronic disease affecting one out of 12 adults in California -- is a growing epidemic that drains the health and economic well-being of families, employers, and communities. The annual health care and related costs of treating diabetes in California in 2012 were about $27.5 billion. However, the California Dept. of Public Health's spending on diabetes prevention has declined due to reductions in its federal funding. In FY 2012-13, California had the lowest per capita funding for diabetes prevention in the nation. One reason is that California does not provide any state funding for diabetes prevention. The Dept. of Public Health has not been able to expand its diabetes prevention to many counties that have a high prevalence of diabetes. This report recommends that state lawmakers fund a grants specialist position to focus on applying for federal and other grants to fight the growing epidemic of diabetes in California.
