The current book, Management of Breast Diseases, is an adaptation of that manual, with Professor Manfred Kaufmann of the Goethe-University of Frankfurt serving as co-editor. Most of the chapters from the original manual have been either extensively revised or discarded, and several new chapters added. This text contains more material than the original manual, but it is still intended as a basic guide for the wide spectrum of clinicians (surgeons, gynecologists, oncologists, radiation onco- gists, internists, general practitioners) who treat breast diseases, both benign and malignant. To compile this text, we assembled experts from throughout the world. Thus, this text provides not only a broad overview of breast diseases, but also highlights diff- ent perspectives from different parts of the world. Yet, it is worth noting that the management of breast cancer is now largely predicated on evidence-based medicine. Several large, randomized prospective trials have demonstrated the ef? cacy of breast cancer screening and chemoprevention. Other large trials have addressed the impact of systemic therapy, radiotherapy, and variations in local therapy on breast cancer mortality. Many of these landmark trials are discussed in this text, and they clearly have had a bene? cial effect. Indeed, since about 1990, breast cancer mortality rates have declined substantially in most industrialized countries, and this trend is expected to continue in the years ahead.

There has been a lot of scientific advances in the radiology, pathology and risk assessment of benign breast lesions since the publication of the first edition. The first edition concentrated on screen-detected lesions, which has been rectified. New symptomatic and screen-detected lesions are discussed in the second edition and include: mastitis and breast abscess, idiopathic granulomatous mastitis, diabetic mastopathy, phyllodes tumour, gynaecomastia and pseudoangiomatous stromal hyperplasia. The chapters on columnar cell lesions and mucocele-like lesions have been extensively updated. Where applicable, genetic analysis of the benign lesions which in breast cancer is becoming part of personalised medicine has been included. The book includes detailed analysis of the main models such as the Gail Model used to assess the subsequent risk of breast cancer in individuals. The current trend in the management of all cancers is preventative. Screening mammography detects early curable cancers as well as indeterminate lesions. These indeterminate mammographic lesions are invariably pathologically benign. The author collated important benign lesions and based on peer-reviewed publications documented the relative risk of subsequent cancer to allow the patient and the clinician to institute preventative measures where possible. This book therefore will be an essential part of multidisciplinary management of patients with symptomatic and screen-detected benign breast lesions.

As we know, the most common breast malignancy is in women; however, benign lesions of the breast are far more frequent than malignant ones. The majority of the information that we have available in the literature about the nonmalignant breast diseases is fragmented in texts and innumerable articles. The information provided in this book throughout its fourteen chapters is very complete with regard to the diagnosis of and indications for the treatment of these diseases. Over the last decade, there have been dramatic advances in the imaging techniques employed for the detection and diagnosis of breast cancer, which now include conventional or antilog mastography; digital ultrasound mammography, and magnetic resonance. The most frequent pathologies of the breast, as observed by those of us devoted to Oncology, are of the benign type. However, this is not so for non-specialist physicians and for the women who suffer from these and for whom they are a true cause for alarm.
