Book excerpt: The economic burden of breast cancer for women under 50 in the United States remains largely unexplored, in part because young women make up a small proportion of breast cancer cases overall. To address this knowledge gap, we conducted a web-based survey to compare data from breast cancer survivors 18–39 years of age at first diagnosis and 40–49 years of age at first diagnosis.We administered a survey to a national convenience sample of 416 women who were 18–49 years of age at the time of their breast cancer diagnosis. We analyzed factors associated with financial decline using multivariate regression.Survivors 18–39 years of age at first diagnosis were more likely to report Stage II–IV breast cancer (P < 0.01). They also quit their jobs more often (14.6%) than older survivors (4.4%; P < 0.01) and faced more job performance issues (55.7% and 42.8%, respectively; P = 0.02). For respondents in both groups, financial decline was more likely if the survivor had at least one comorbid condition (odds ratios: 2.36–3.21) or was diagnosed at Stage II–IV breast cancer (odds ratios: 2.04–3.51).

Due to the combination of improved treatments and the benefits of mammography screening, breast cancer mortality has decreased steadily since 1989. Yet breast cancer remains a major problem, second only to lung cancer as a leading cause of death from cancer for women. To date, no means to prevent breast cancer has been discovered and experience has shown that treatments are most effective when a cancer is detected early, before it has spread to other tissues. These two facts suggest that the most effective way to continue reducing the death toll from breast cancer is improved early detection and diagnosis. Building on the 2001 report Mammography and Beyond, this new book not only examines ways to improve implementation and use of new and current breast cancer detection technologies but also evaluates the need to develop tools that identify women who would benefit most from early detection screening. Saving Women's Lives: Strategies for Improving Breast Cancer Detection and Diagnosis encourages more research that integrates the development, validation, and analysis of the types of technologies in clinical practice that promote improved risk identification techniques. Measures of incidence, survival and mortality are critical to the interpretation of data on progress in the fight against cancer, and in the evaluation of the overall effectiveness of cancer control programmes. Randomized controlled clinical trials have shown many modern protocols for cancer treatment to be more effective than earlier treatments, but until now, comparable population-based survival figures have raraly been available. EUROCARE is a concerted action among European cancer registries, aimed at estimating and comparing the survival of cancer patients in different European populations. The rationale behind this project is to optimize the comparability of survival data by using an agreed and standard definition of the diseases for which survival is to be compared, and by taking due account of basic demographic variables. The EUROCARE Working Group succeeded in collecting, checking and editing data on cancer survival from 30 cancer registries in 12 countries, and established a database covering some 800,000 cancer patients in the period 1978-85 and followed up to the end of 1990. This database forms the raw material for this monograph.

Book excerpt: The results of randomized trials evaluating the use of early or adjuvant systemic treatment for patients with resectable breast cancer provide an eloquent rebuttal to those who would argue that we have made no progress in the treatment of cancer. Many of the tumors that we have been most successful in curing with chemotherapy and other newer forms of treatment are relatively uncommon. In contrast, breast cancer continues to be the single most common malignancy among women in the western world, is increasingly a cause of death throughout Asia and Third-World countries, and remains one of the most substantial causes of cancer mortality world wide. The use of mammography as a means of early detection has been shown to reduce breast cancer mortality by 25-35% among those popu lations in which it is utilized. The use of adjuvant systemic treatment in appropriate patients provides a similar (and additional) reduction in breast cancer mortality. Few subjects have been so systematically studied in the history of medicine, and it seems fair to conclude that the value to adjuvant systemic therapy in prolonging the lives of women with breast cancer is more firmly supported by empirical evidence than even the more conventional or primary treatments using various combinations ofsurgery and radiotherapy.
