The literature report a majority of persons who are lesbian, gay, bisexual, and transgender (LGBT) have experienced discrimination from a healthcare provider. This study encouraged nurses to assess their beliefs and behavior when providing health services to lesbian and gay individuals. Such self-assessment can positively impact patient outcomes. Nurses acknowledging sexual orientation and providing a safe environment for the patient and their family improves outcomes by providing patient-centered care.Aim. To assess their beliefs and behavior when providing healthcare services to gay and lesbian individuals using the Gay Affirmative Practice (GAP) scale.Methods. The study involved convenience sampling of registered nurses in a southwest urban hospital system utilizing an eNewsletter with a link to a Qualtrics survey that included a consent statement, demographic questions, and the Gay Affirmative Practice (GAP) Scale. The data was analyzed using descriptive statistics.Findings. Of 70 respondents, 89% were female, 91% Caucasian, 90% heterosexual, and 84% Christian. The average age was 49; 76% had a baccalaureate or master’s degree; 71% had a nursing certification; 60% had a friend or family member who is gay or lesbian; and 61% believed a cultural competence course would benefit them professionally. The mean GAP score was 110 (150 possible); the average belief score was 63 (75 possible); and the average behavior score was 49 (75 possible).Conclusions. Small sample size limited generalizability. Belief scores were higher than behavior scores, likely representing nurses’ positive beliefs and their requirement for education to increase behavior scores. Recommendations include future research with larger sample sizes throughout the nation; development of a tool that has language more reflective of nursing; and promote cultural competence courses with definitions of LGBT terminology and how to ask sexual orientation- and gender identity- questions in a safe and optimal manner.

Book excerpt: The general aim of the project was to describe the situation of lesbians and gay men in a nursing environment by studying the attitudes of nursing staff and students, and the experiences of gay nursing staff in their work environment and of gay patients and partners in their encounters with nursing. The study for papers I and II had a descriptive, comparative design. Nurses, assistant nurses, and nursing students completed the Attitudes Toward Homosexuality Scale (ATHS), the Causes of Homosexuality Questionnaire (CHQ), and the Affect Adjective Checklist (AAC), along with the Nursing Behaviour Questionnaire (NBQ). In general, the participating nursing staff and students expressed positive attitudes, though some subjects reported very negative attitudes toward gay people. The participants also expressed a full spectrum of emotions from delight to anger. The studies for papers III and IV had a descriptive, explorative design. Here, the data collection was performed by semi-structured interviews. The informants were gay nursing staff, gay patients and partners of patients. The gay personnel reported fear and concern about heterosexuals' reactions when these informants "come out," and were constantly assesing the risk being open about their sexual orientation among their workmates. Nearly all patients and partners described several situations where heteronormative assumptions were communicated by the nursing staff. Almost all informants offered recommendations for nursing staff to facilitate communication. According to the presented findings, probably only a small minority in Swedish nursing have negative attitudes toward lesbians and gay men. However, the informants told of heteronormative language and behaviour in nursing contexts that were percieved as insensitive, insulting and humaliating. Nursing staff need to learn how to communicate in a more natural way and to be aware of the norms they communicate through the language and behaviour they use.

Nurses who express negative biases in the delivery of care influence many of the disparities facing this vulnerable patient population. Educational interventions that are focused on increasing nurses' knowledge of LGBT healthcare needs have proven effective in improving patient compliance and enhancing patient care outcomes. This evidence-based practice change project was designed to educate nurses (n=25) on the healthcare needs of the LGBT population. The project included a pretest and posttest with an educational intervention. The project manager, in collaboration with subject matter experts, developed a questionnaire, which consisted of 15 knowledge and five attitude questions. The questionnaire was administered before and after an educational intervention to measure any changes in nurses' knowledge and attitudes toward LGBT patients. Findings indicated that knowledge scores increased from a baseline of 69.1%, which was below the 80% benchmark, to a post intervention score of 97.6%, which was above the intended benchmark. The average mean scores measuring attitudes increased significantly, t = 2.06 and p = .04. The findings of this evidence-based practice change project have significant implications on the effects of educating nurses on the healthcare needs of the LGBT population, that dramatically influence practice and patient care outcomes. A cornerstone of the gay rights movement, equality in the workplace has been a pivotal struggle for gays and lesbians. This study examined the attitudes and opinions of registered nurses (RNs) regarding homosexuals in general and the protection of homosexuals in the workplace through a nondiscrimination policy. The author measured overall homophobic and discriminatory beliefs of the sample using the Attitudes Toward Lesbian and Gay Men (ATLG) Scale; the demographic questionnaire was infused with questions regarding a protective workplace policy. Using T-tests, one-way analysis of variance (ANOVA) and structural equation modeling (SEM), correlations between independent variables (gender, age, religious association, belief in the "free choice" model of homosexuality, education level, exposure to homosexuals through friends and/or family associations, race/ethnicity, and support or non-support of a workplace nondiscrimination policy protective of gay men and lesbians) with the dependent variable of homophobia were explored.
