La mandibule étant une pièce osseuse mobile, le clinicien doit pouvoir, à tout moment d'un diagnostic occlusal ou d'un traitement, situer la mandibule par rapport au maxillaire. Il doit donc déterminer, parmi les différentes positions mandibulaires, une position idéalement physiologique, reproductible, répétitive et non affectée par le traitement envisagé, c'est-à-dire une position de référence. Parmi les trois référentiels de l'appareil manducateur (système dentaire, ostéo-articulaire et musculaire), seules deux positions répondent au cahier des charges d'une position de référence : une situation définie par les contacts occlusaux, c'est l'occlusion d'intercuspidie maximale ; une situation définie par les articulations temporo-mandibulaires, c'est la relation articulaire de référence. La connaissance des caractéristiques physiologiques et pathologiques de ces positions de référence est nécessaire afin de connaître les critères de choix qui en découlent et ceci en fonction des différentes situations cliniques rencontrées.

Dans un premier temps, l'étude des définitions de l'intercuspidie maximale, de la relation centrée et des positions thérapeutiques est réalisée en se réferant à la bibliographie. Elle met en avant certaines discordances entre les différents auteurs et notamment d'ordre sémantique. Lors de la mise en œuvre d'une réhabilitation prothétique, le praticien se voit confronté au choix de la position thérapeutique, et donc à la technique d'enregistrement de celle-ci. Ces différents points sont étudiés dans un second chapitre. La prise en compte de l'influence de la posture corporelle sur la position mandibulaire est une notion récente qui commence à être développée par certains auteurs. Ce point est également évoqué à la fin de cet ouvrage.
