Book excerpt: Le remplacement des dents absentes par des implants est devenu un acte courant en dentisterie. Les technologies d'aide à la pose d'implants se multiplient avec la diversification des matériaux et des protocoles. De même que les logiciels pour la modélisation et l'impression 3D de prothèses unitaires ou plurales implanto-portées. Ces logiciels et leurs outils associés permettent une continuité dans la préparation des réhabilitations orales implantaires ; continuité décrite sous le nom de flux numérique de travail (FNT). Après de brefs rappels sur les généralités de cette technique, ce travail de thèse se propose de questionner la littérature sur la reproductibilité de l'utilisation du FNT pour la chirurgie implantaire et la fabrication de prothèses implanto-portées tout en le comparant aux méthodes classiques. Une recherche numérique dans la base de données Cochrane a mené à la sélection de 37 articles qui ont été analysés. Les résultats ont été subdivisés en trois sous-parties pour les résultats de la chirurgie guidée selon le type d'appui du guide (dentaire, muqueux, osseux). La partie sur la prothèse implantaire comportait une première sous-partie dédiée à l'utilisation des corps de scannage, une seconde concernant l'adaptation marginale et interne des restaurations issues du FNT et une dernière sur les ajustements cliniques le jour de la pose. Les chirurgies guidées ont présenté des résultats similaires voire meilleurs que les chirurgies dites à main levée. Elles présentent des résultats significativement différents suivant les études entre elles. Les résultats sont aussi significativement différents au sein des publications elles-mêmes. Les protocoles de chirurgies guidées abordés étaient hétéroclites, de même que le recrutement des patients. Aux vues des résultats obtenus les chirurgies guidées sont des protocoles fiables et aux résultats cliniques acceptables mais ne sont pas reproductibles. En prothèse, l'utilisation des corps de scannage est encore mal connue. Les résultats montrent des différences significatives avec l'utilisation de CS lors de prises d'empreintes optiques. On voit clairement une supériorité en termes d'adaptation marginale et interne des prothèses plurales issues de la méthode classique. Les couronnes unitaires issues du FNT ont besoin de significativement moins d'ajustements cliniques que celles issues de la méthode classique. Les ajustements cliniques présentent des résultats similaires suivant les études pour les couronnes issues du FNT entre elles. Les prothèses issues du FNT présentent des caractéristiques cliniques fiables cliniquement mais très peu reproductibles. En conclusion les auteurs s'accordent à dire que le FNT est une avancée technologique permettant d'obtenir des résultats cliniquement équivalents voir meilleur en comparaison de la méthode classique au niveau chirurgical et prothétique. Faisant du FNT une méthode précise mais dont la reproductibilité n'est toujours pas atteinte, ce qui constitue un des enjeux de la dentisterie de demain.

Cette thèse porte sur la caractérisation physico-chimique et l'évaluation biologique in vitro de deux différents implants dentaires. Nous avons investigué un implant minimalement invasif nouvellement sur le marché en comparaison avec un implant témoin. Nous avons réalisé, d'une part la caractérisation précise des surfaces implantaires en termes de design, de topographie et de composition chimique et d'autre part, l'effet de ces surfaces sur le comportement biologique de deux types cellulaires représentatives de la sphère orale. La finalité de cette thèse est de prédire la capacité d'ostéo-intégration et le potentiel biologique in vivo des implants étudiés. Tous les aspects abordés lors de la discussion ont été appuyés à l'aide de la littérature scientifique actuelle. D'autres études concernant les propriétés mécaniques des implants minimalement invasifs et l'apport d'analyses en immunohistochimie in vivo permettront de mettre en lumière les propriétés cliniques d'ostéo-intégration de ces implants.
