Book excerpt: Les structures et compositions des tissus dentaires (émail, cément, dentine coronaire et radiculaire) sont similaires à celles du tissu osseux. Il a ainsi été proposé d'utiliser les tissus dentaires comme matériau de substitution osseuse. D'abord lors de chirurgies de préservation alvéolaire puis dans toutes les indications classiques d'élévation sinusienne, de régénération osseuse guidée, de reconstitution de crête ou encore de comblement de lésions osseuses d'origine endodontique ou parodontale. Le but de cette revue de la littérature est de répondre à la question : « Les résultats cliniques en termes de gain osseux, de stabilité implantaire et de morbidité lors de greffes de tissus dentaires autologues sont-ils cliniquement identiques à ceux de matériaux conventionnels de substitution osseuse dans le cadre de traitements implantaires ? ». Les publications sélectionnées devaient comparer la greffe de tissus dentaires autologues avec au moins un autre matériau, selon le même protocole chirurgical. Seulement dix publications ont été retenues. L'analyse a montré une grande hétérogénéité des données recueillies et notamment de composition du greffon dentaire autologue. Néanmoins, celui-ci semble présenter une biocompatibilité au moins équivalente à celle des autres biomatériaux conventionnels d'apport ou de comblement osseux. Malgré des résultats prometteurs en termes de gains de volume osseux, de stabilité implantaire et de morbidité associée, la grande variabilité méthodologique des publications retenues ne permet pas de conclure sur l'efficacité relative de ce biomatériau en tant que substitut osseux.

La réduction du volume alvéolaire conduit à des problèmes thérapeutiques et esthétiques. La conservation du volume osseux après extraction constitue donc un objectif prioritaire à atteindre, et l’utilisation de matériaux de substitution osseuse dans ce cadre paraît a priori tout à fait justifiée. Afin de palier cette résorption osseuse, de nombreux matériaux de reconstruction osseuse ont vu le jour. Il s'agit d'allogreffe (greffe des tissus d’un individu à un autre d’une même espèce), de xénogreffe (greffe des tissus d’un individu à un autre d’une autre espèce) ou de matériau synthétique. La multiplicité de ces matériaux témoigne de l'énorme demande des praticiens mais prouve également que le matériau de substitution osseuse idéal n’existe pas. Cette ouvrage a pour but d'évaluer l'efficacité des matériaux de reconstruction osseuse dans les alvéoles d'extraction. De plus, nous mettrons en évidence, s'il en existe, les différences significatives, histologiques et cliniques, entre les différents matériaux.
