Ils risquent davantage de souffrir de problèmes de santé physique, mentale et d'être isolés socialement comparativement à la population générale ou à d'autres aidants. Bien que plusieurs interventions aient été développées pour améliorer le bien-être des proches aidants telles que les groupes psychoéducatifs, peu se sont avérées efficaces. Devant ce constat, l'importance d'implanter et de conceptualiser d'autres modèles d'intervention demeure actuelle. Cette étude vise à évaluer l'efficacité d'une intervention offerte à des aidantes d'une personne atteinte de démence à l'aide d'un devis préexpérimental. L'intervention proposée, axée sur la régulation des émotions, se déroule en petit groupe et s'échelonne sur 13 rencontres hebdomadaires de 90 minutes. Le chapitre I expose le contexte général de cette thèse et se veut une mise à jour de l'état des connaissances entourant le rôle d'aidant ainsi qu'une revue critique des études d'intervention effectuées auprès de cette clientèle. Le chapitre II présente les résultats concernant l'efficacité de l'intervention élaborée. Les données recueillies proviennent de 23 aidantes d'un proche atteint de démence ayant complété l'intervention. Globalement, les résultats permettent de constater une utilisation plus fréquente par les aidantes des stratégies adaptées de la régulation des émotions suite à l'intervention, une diminution significative de leur détresse psychologique et un niveau de réactivité plus faible face aux troubles de comportement. Afin de mieux comprendre ces effets, le chapitre III présente l'évaluation qualitative des processus de l'intervention. Pour ce faire, des entretiens semi-dirigés ont eu lieu avant et après l'intervention avec 24 aidantes ayant participé au programme. L'analyse de contenu révèle que plusieurs changements positifs dus à l'intervention sont perçus par les aidantes. Les principaux facteurs contribuant à l'émergence de ces changements ont été documentés. Finalement, le chapitre IV discute et critique l'ensemble des résultats obtenus en mettant en lumière certaines considérations méthodologiques et cliniques.

Book excerpt: Introduction : l'impact d'une intervention éducative pure sur le risque et le délai de placement en institution du patient atteint de maladie d'Alzheimer (MA) a rarement était étudié. Objectif : étudier l'effet d'une intervention éducative adressée au couple aidant-aidé sur le placement en institution des patients atteint de MA ayant participé à l'essai THERAD et identifier les facteurs prédictifs de placement. Méthode : THERAD est un essai interventionnel monocentrique contrôlé randomisé. Les patients à un stade léger à modérément sévère de MA vivant à domicile et leur aidant principal ont été recrutés sur le centre mémoire du CHU de Toulouse et suivis sur 12 mois. L'intervention était un programme d'ETP de 6 séances (2 individuelles et 4 collectives) entre M0 et M2. Notre critère de jugement principal était le placement en institution du patient (et sa date) à 2 ans de suivi. Le recueil était prospectif jusqu'à M12, puis rétrospectif à M24. Nos critères de jugement secondaires étaient les caractéristiques des couples aidant-aidé recueillies dans THERAD. Une analyse bivariée a été menée avec un modèle de Cox pour chaque variable, avec le temps entre inclusion et placement comme variable dépendante. Résultats : 196 couples ont été inclus dans THERAD (98 dans le groupe intervention et 98 dans le groupe contrôle) entre janvier 2013 et septembre 2015. 49 patients (25,1%) étaient institutionnalisés à 2 ans. Le risque de placement en institution à 1 an et à 2 ans de suivi ne différait pas entre les deux groupes (HR à 1 an = 0,56, IC=[0,28;1,12], p=0,0953 ; HR à 2 ans = 0,77, IC=[0,44;1,36], p=0,3721). Les variables associées à une augmentation du risque de placement étaient pour le patient : l'avancée en âge, l'absence de vie maritale, l'ancienneté du diagnostic, la détérioration cognitive, la perte d'autonomie sur les IADL et les ADL, les symptômes psycho-comportementaux de la démence (en particulier la désinhibition, les comportements moteurs aberrants, les idées délirantes, l'agressivité), la prise isolée d'un traitement antidépresseur, la fréquentation d'un accueil de jour ; pour l'aidant : le statut d'enfant non cohabitant, un niveau d'études élevé, le niveau de fardeau. Conclusion : le programme éducatif de THERAD n'a pas eu d'effet précoce ni prolongé sur l'institutionnalisation. Plusieurs facteurs prédictifs de placement ont été mis évidence. Une analyse multivariée doit être menée. Des interventions multimodales prolongées englobant l'éducation thérapeutique, structurées en fonction des besoins du couple, pourraient être plus efficaces. D'autres études seront nécessaires.
