This report summarizes key learnings from 45 projects in the Fund's home care theme area. Introductory sections set the context for the projects (including background on home care, provincial policies, & other issues facing home care in Canada) and provide an overview of the Fund's home care studies. The report then discusses significant & relevant findings of projects that examined the cost-effectiveness of home care, improving access, barriers to appropriate use, substitution of hospital services with home care, reforming existing programs, health human resources, & other issues. Finally, implications of the findings for policy & practice are discussed. The appendix summarizes information on the Fund projects relevant to home care.

Book excerpt: Les proches, ou aidants naturels, sont indispensables à l'accompagnement d'une personne en fin de vie à domicile. Nous avons donc recherché leurs besoins, par des entretiens semi-dirigés auprès de quinze personnes ayant accompagné un patient suivi en soins palliatifs par l'Hospitalisation A Domicile (HAD) de Dax. D'après notre étude, les besoins les plus importants, selon ces proches, sont : les soins apportés au mourant avec bien sûr le soulagement de ses symptômes et en premier lieu de la douleur ; la coordination des différents soignants ; et l'accompagnement fait au quotidien par les infirmières et aides-soignantes. Le dispositif de permanence 24h/24 est aussi primordial pour se sentir en sécurité à domicile. De plus les proches doivent être considérés comme des partenaires de la relation de soins en fin de vie, et donc intégrés aux décisions et aux soins eux-mêmes s'ils le souhaitent. Nous, professionnels de santé, devons être attentifs aux proches, car le retentissement de cet accompagnement n'est pas anodin. Il entraîne des répercussions sur leur vie professionnelle, personnelle, familiale et sur leur santé. Il est donc important de leur consacrer du temps afin d'entendre leur souffrance et de les préparer à la mort de leur parent, pour qu'ils puissent être présents auprès de leur malade et vivre ses derniers moments avec lui jusqu'au bout. La pluridisciplinarité de l'accompagnement d'un patient en soins palliatifs et de ses proches est cruciale. Le développement des HAD ainsi que des réseaux est donc préconisé. Les formations de tous les soignants sont aussi essentielles. Et une transmission des savoirs aux proches nous semble recommandée.
