Book excerpt: L'avènement de la nasofibroscopie dans les années 80 a réduit la pratique de l'endoscopie au tube rigide, sous anesthésie générale. Objectifs : Evaluer les apports de l'endoscopie rigide et ses indications. Matériel et méthode : Etude rétrospective de tous les patients de un an ou moins, bénéficiant d'une nasofibroscopie pour un symptôme respiratoire entre janvier 2003 et décembre 2009. Les enfants ayant une nasofibroscopie puis une endoscopie rigide, sont comparés aux enfants ayant une nasofibroscopie seule, du point de vue clinique et de leurs critères de gravité. Trois types de critères de gravité sont définis : respiratoire, alimentaire et association des deux. Résultats : La moitié des enfants (47 %) ayant une nasofibroscopie seule, n'ont pas de signe de gravité dans le motif de consultation. Les enfants ayant les deux examens ont un signe de gravité pour 74 % d'entre eux, au moment de l'indication de l'endoscopie sous anesthésie générale. Les indications de l'endoscopie sont de trois types : l'exploration d'une lésion vue en nasofibroscopie avec possible geste thérapeutique associé (37 %), la présence d'un critère de gravité (44 %) et l'exploration nasofibroscopique incomplète (19 %). Dans 30 % des cas, l'examen sous anesthésie générale n'apporte pas de nouvelle information, 70 % de ces enfants ont un signe de gravité. Pour les 70 % autres, l'endoscopie confirme une lésion vue (28 %), révèle une nouvelle lésion (30 %) ou redresse le diagnostic (12 %). La fréquence des enfants présentant des lésions multiples du tractus respiratoire, est évaluée à 35 % (10 % de la population totale). Conclusion : Il n'y a pas d'indication à la réalisation systématique de l'endoscopie sous anesthésie générale dans la dyspnée de l'enfant de moins de un an. Une lésion vue, la présence d'un critère de gravité et l'exploration incomplète à la nasofibroscopie sont les 3 principales indications d'une endoscopie rigide sous anesthésie générale.
