La première cause d'échec de contraception est l'oubli de pilule. Pourtant de nouveaux moyens de contraception permettant une meilleure observance existent. Le but de l'étude était d'étudier les connaissances et les habitudes de prescription par les médecins généralistes des méthodes alternatives à la contraception orale. Il s'agit d'une étude épidémiologique observationnelle prospective : enquête postale par auto-questionnaires auprès d'un tiers des médecins généralistes du Maine-et-Loire. Le taux de réponse était de 44%, 135 questionnaires ont pu être exploités. Notre échantillon était représentatif en sexe et en âge avec la démographie médicale régionale. 45 médecins soit un tiers prescrivaient l'implant. Les médecins de moins de 50 ans prescrivaient significativement plus que leurs aînés (p = 5.10 - 5), en particulier chez les femmes (p = 0.02). 40 des 135 répondants soit 30% prescrivaient l'anneau vaginal et la patch transdermique. Les médecins femmes sont en proportion plus nombreuses que leurs confrères masculins à prescrire le patch et l'anneau. Et ce sont les médecins de plus de 50 ans qui prescrivent le moins ces méthodes. Seulement 48 soit 36% accompagnaient leur prescription d'une ordonnance de contraception d'urgence. Ces nouvelles méthodes sont donc peu connues et peu prescrites par les généralistes. Les formations sur la contracpetion doivent donc continuer à se renforcer au cours des études et pendant l'activité des médecins généralistes avec pour objectifs : une meilleure prescription de la contraception, une diminution des grossesses non prévues et donc des demandes d'IVG.

Book excerpt: Contexte : Depuis les années 2000, on assiste en France à une véritable diversification de l'offre des méthodes contraceptives à destination des femmes leur permettant notamment une meilleure observance, cependant bien que la pilule soit corrélée au risque important d'oubli et à un taux d'échec stable cette dernière reste le moyen le plus prescrit. L'objectif est de mettre en évidence les raisons pour lesquelles le paysage contraceptif français reste pilulo-centré Méthode : Étude descriptive, multicentrique et transversale, réalisée à l'aide de questionnaires anonymes standardisés déposés dans les salles d'attente de 20 cabinets de médecine générale du Nord-Pas-De-Calais. Les données ont été collectées du 24 janvier 2022 au 8 Mars 2022. Le critère d'exclusion était : tout sujet déclarant ne pas être actuellement sous contraception. Parmi les sujets sous contraception, les caractéristiques de ceux actuellement sous pilule ont été comparées à celles des sujets sous autre mode contraceptif. L'ensemble des analyses a été réalisé sous le logiciel R version 4.1.0 avec un seuil de significativité fixé à 5 % Résultats : Étude ayant inclus 67 femmes. Les résultats ont montré que la prescription d'une contraception orale pouvait être favorisé par le passé contraceptif des femmes qui pour la majorité ont démarré leur vie contraceptive par la pilule (45(70%)) puis l'ont garder par la suite sans forcément la remettre en question par manque de connaissance sur les autres modes qui seraient susceptibles de mieux leur convenir. On observait également l'influence des contraintes financières relatives au coût de certaines méthodes qui ne sont pas ou peu remboursées par la Sécurité sociale et qui peuvent constituer un obstacle pour certaines femmes, en particulier les plus jeunes, ainsi les femmes qui sont études étaient celles qui avaient le plus recours à la pilule contraceptive. Par ailleurs les bénéfices non contraceptifs de la pilule (amélioration de l'acné, de la dysménorrhée..) expliquaient également la popularité de cette dernière puisque parmi les femmes sous pilule, 37% déclaraient avoir choisi ce mode de contraception pour des raisons autres que strictement contraceptives. Conclusion : Pour minorer le taux d'échec contraceptif relatif à la pilule il est important que le recours à cette dernière soit le reflet d'un choix délibéré et adapté des femmes. Pour tendre vers cet objectif il faut optimiser la diffusion d'informations par les professionnels de santé sur l'ensemble du panel contraceptif, et limiter les contraintes financières d'accessibilité à tous les moyens de contraception disponible. Par ailleurs, le remplacement des hormones synthétiques par des hormones naturelles pourrait s'avérer positif du point de vue des femmes, en effet ces dernières sont aujourd'hui à la recherche d'un contraceptif fiable, naturel, et avec le moins d'effets secondaires possibles.
