En 2015, 6,2 millions d'immigrés vivaient en France et 7,3 millions de personnes nées en France avaient avaient au moins un parent immigré, mais les aspects transculturels ne sont pas toujours pris en compte dans la compréhension de certains comportements chez les parents et chez les adolescents, dans l'institution scolaire. Notre travail a voulu déterminer le lien entre le refus scolaire anxieux rencontré chez les adolescents en situation transculturelle et leur construction identitaire. Pour cette étude qualitative, les participants devaient avoir plus de 12 ans, être pris en charge depuis plus de 6 mois pour un refus scolaire anxieux et être nés à l'étranger ou avoir l'un de ses deux parents nés hors France métropolitaine. Le refus scolaire anxieux chez les adolescents en situation transculturelle, nous apparaît être le symptôme d'une suspension des processus psychiques en cours lors de l'adolescence sur une butée probable de la question de l'identité. L'adolescent ne parvient pas à s'insérer dans la vie sociale. Il n'arrive pas non plus à assumer son désir d'autonomie face à l'ampleur des conflits de loyauté dans lesquels il se sent piégé. Et enfin, le processus de métissage qui a lieu lors de l'adolescence est pour le moins en échec. Nous pensons, qu'en plus des thérapeutiques utilisées dans le refus scolaire anxieux, les groupes de pairs et les approches sociologique et transculturelle permettent d'accompagner les adolescents en situation transculturelle à se penser et à retouver le plaisir de penser.

Si de nombreuses études ont cherché les facteurs familiaux de ce trouble, très peu se sont concentrées sur le vécu des parents, et aucune ne s'est intéressée à ces familles lorsqu'elles sont issues de l'immigration de manière spécifique. Il s'agit d'une étude qualitative transversale, observationnelle et non interventionnelle. Le recueil de données s'est fait au moyen d'entretiens semi-directifs analysés selon la méthode phénoménologique interprétative. Onze entretiens ont été réalisés. Les résultats ont été organisés autour de 4 thèmes principaux : le bouleversement des représentations parentales à la lumière du refus scolaire ; le refus scolaire comme un échec du mandat de réussite familial ; les représentations de l'école entre idéalisation, méfiance et déception ; les solutions au refus scolaire, représentations des soins et autres solutions. La déscolarisation de leurs enfants amène les parents à interroger leurs modèles familiaux et leurs choix parentaux. Elle questionne le rapport des parents migrants à l'institution scolaire française, l'héritage de l'histoire collective et l'héritage de l'histoire familiale. D'un point de vue transculturel, le refus scolaire anxieux à l'adolescence peut être vu comme une difficulté à gérer le contact entre la famille et l'école, au moment où la construction identitaire nécessite de négocier entre ces deux mondes, un échec du métissage. Cette étude illustre la nécessité de prendre en considération la culture des parents et l'histoire migratoire dans la prise en charge du refus scolaire anxieux.

Pourtant, le diagnostic et la prise en charge des troubles dépressifs chez ces sujets sont retardés du fait d'une symptomatologie dépressive mal définie et d'un métissage culturel ayant un impact sur l'expression symptomatique et la présentation clinique des troubles mentaux. L'objectif de cette étude est d'étudier l'expérience de la dépression chez les adolescents enfants de migrants, de préciser les mécanismes et les modalités d'expression de cette souffrance en lien avec les vulnérabilités spécifiques à cette population. Huit entretiens semi structurés ont été réalisés. L'analyse est phénoménologique, utilisant l'IPA (Interpretative Phenomeno-logical Analysis). Résultats : L'analyse thématique phénoménologique des entretiens a permis de distinguer 12 thèmes organisés en 4 axes d'expérience : l) La dépression incertaine : de la souffrance invisible à la souffrance visible, évoque l'incertitude de la dépression et la nécessité de rendre visible une souffrance interne difficilement descriptible. 2) A la recherche de sens et de mise en lien, précise la difficulté pour les adolescents à faire du lien, le besoin de trouver un sens à leur souffrance, besoin alimenté par un vécu d'illégitimité et un défaut de transmission, la présence de non-dits et secrets. 3) Incompréhensions et conflits intergénérationnels, explore l'incompréhension parentale de la souffrance de leur enfant et le rejet des symboles de la culture des parents. 4) Clivages : dedans et dehors, ici et là-bas, décrit les conflits entre le monde du dedans et du dehors, entre le pays hôte et le pays d'origine. Discussion-Conclusion : L'analyse des résultats suggère une incertitude pour les participants sur le fait d'avoir été ou d'être déprimés et une difficulté à lier leur vécu au concept de dépression du fait d'une absence au moins partielle d'une grille de lecture permettant de penser la dépression. L'expression de la souffrance se fait par l'intermédiaire du corps et de troubles du comportement, qui semblent avoir une fonction de communication, de représentation et de résolution des conflits internes issus des vulnérabilités spécifiques des enfants de migrants. Du fait d'un vécu de différence à l'adolescence, il existe une résurgence du récit migratoire, de la séparation et des pertes en lien avec la migration, s'intégrant dans le processus de séparation-individuation. Leur adolescence semble être le théâtre d'une migration associée à un vécu de double perte qui pourrait favoriser l'émergence d'un sentiment de culpabilité, de dévalorisation et d'affects dépressifs. Il existe chez les participants une difficulté à raconter et à imaginer les récits familiaux et migratoires qui semble être en lien avec des défauts de narration et de transmission. Les sujets rapportent un vécu d'illégitimité à souffrir qui provient de la comparaison avec le vécu parental, de la non reconnaissance de la souffrance et de la consigne de non légitimité à souffrir adressée par les parents. Le vécu de culpabilité est également entretenu par la « dette migratoire » qui s'inscrit dans l'inconscient des enfants de migrants. La résurgence du récit migratoire pourrait permettre une relance de la transmission de l'histoire et du récit familial. Une thérapie à la fois individuelle et familiale centrée sur la narrativité pourrait accompagner et faciliter le processus de relance narrative.
