Book excerpt: L'objet de notre thèse est de démontrer qu'au-delà de l'importance du diagnostic précoce dans la résolution clinique des troubles psychiques de l'ESPTT, d'autres déterminants essentiels contribuent, par leur synergie, à cette résolution. Le Trouble de Stress Post-Traumatique (TSPT) est un trouble psychique issu de la confrontation d'une personne à un événement appréhendé comme une mise en danger, la mort ou son récit faisant effraction en un instant traumatique invisible. Ce n'est que secondairement, après une période de latence, que ce syndrome apparait, s'exprimant par des symptômes spécifiques (reviviscences, évitement anxiophobique, hyper-vigilance). Son évolution est aléatoire, faite d'accalmie et de résurgences, de guérison ou de chronicisation, mais souvent défavorable en l'absence de prise en charge. Sur le plan méthodologique, nous avons procédé à l'exploitation statistique de 38 questionnaires en nous référant à la grille PCLS créée en 1993 par Weathers F.W. et al., selon les critères diagnostiques du DSM IV. Nous avons mené 22 entretiens auprès de Policiers ayant eu un ESPT. Les résultats de nos travaux confirment l'efficience évidente d'un diagnostic précoce d'ESPT sur la souffrance psychique. Ils soulignent par ailleurs l'impact d'autres déterminants sur la reconstruction, aux premiers rangs desquels figurent la dimension familiale et les reconnaissances institutionnelles de leur blessure. Nos travaux rendent compte, également, de la nécessité impérieuse d'une sensibilisation de l'ensemble des personnels quant aux symptômes spécifiques de ce syndrome, facilitant le partage d'informations entre la hiérarchie directe et les différents acteurs médicaux psycho sociaux du Ministère dans cet état de veille des états d'alerte du traumatisme. Ils démontrent enfin l'importance de la traçabilité : elle favorise, en amont, le dépistage et le diagnostic de ces symptômes par les professionnels de santé, et elle permet, au plan de l'imputabilité, l'établissement formel du lien certain, direct entre l'évènement déclencheur et l'apparition de ces symptômes.
