Book excerpt: Tant par leur fréquence que par leurs répercussions humaines, sociales et économiques, les troubles du sommeil constituent un important problème de santé publique. Et alors que la consommation d'hypnotiques connaît un essor croissant une partie importante des personnes présentant une insomnie n'en parlent pas à leur médecin et ces derniers à leur tour méconnaissent fréquemment ces troubles chez leurs patients. Cette étude a pour objectif principal d'évaluer le sommeil des patients de médecine générale. Méthode - 241 patients adultes de médecine générale ont été inclus dans une étude en répondant à un questionnaire anonyme. Les questions concernaient les caractéristiques de leur sommeil, leur mode de vie et leur prise médicamenteuse éventuelle. Résultats - On constate alors que plus de la moitié des patents de I 'étude sont insatisfaits de leur sommeil. Ce sont principalement des femmes qui sont significativement plus concernées (au risque d'erreur de 5%) et on observe que les troubles augmentent généralement avec l'âge. Certaines caractéristiques et comportements ont également pu être mis en cause de manière significative, comme l'obésité, la consommation tardive d'excitants, la sédentarité ou des horaires de vie décalés. 39.8% des patients se plaignant de leur sommeil consomment des traitements (majoritairement des benzodiazépines) et seulement 25% d'entre eux observent une régression totale de leur trouble sous traitement. Les méthodes non médicamenteuses pouvant intervenir dans la prise en charge des insomnies sont par ailleurs très peu connues des sujets de l'étude. Conclusion - Le médecin, au centre de la prise en charge des troubles du sommeil, doit s'efforcer de dépister et caractériser ces troubles, d'éliminer les facteurs différentiels et de corriger les mauvaises habitudes de vie des patients. Il doit également réserver la prescription d'hypnotiques aux rares cas où ils en ont l'indication.

Book excerpt: The Violence of Modernity turns to Charles Baudelaire, one of the most canonical figures of literary modernism, in order to reclaim an aesthetic legacy for ethical inquiry and historical critique. Works of modern literature are commonly theorized as symptomatic responses to the trauma of history. In a climate that tends to privilege crisis over critique, Debarati Sanyal argues that it is urgent to rethink literary experience in terms that recall its contestatory potential. Examining Baudelaire's poems afresh, she shifts the focus of critical attention toward an account of modernism as an active engagement with violence, specifically the violence of history in nineteenth-century France. Sanyal analyzes a literary current that uses the traditional hallmarks of modernism—irony, intertextuality, self-reflexivity, and formalism—to challenge the historical violence of modernity. Baudelaire and the committed ironists writing in his wake teach us how to read and resist the violence of history, and thereby to challenge the melancholy tenor of our contemporary "wound culture." In a series of provocative readings, Sanyal presents Baudelaire's poetry as an aesthetic form that contests historical violence through rhetorical strategies of complicity, counterviolence, and critique. The book develops a new account of Baudelaire's significance as a modernist by dislodging him both from his traditional status as a practitioner of "art for art's sake" and from his more recent incarnation as the poet of trauma. Following her extended analysis of Baudelaire's poetry, Sanyal in later chapters considers a number of authors influenced by his strategies—including Rachilde, Virginie Despentes, Albert Camus, and Jean-Paul Sartre—to examine the relevance of their interventions for our current climate of trauma and terror.
