Book excerpt: Malgré les programmes de prévention instaurés dans la plupart des pays industrialisés, la maladie carieuse reste la pathologie bucco-dentaire ayant la plus haute prévalence dans le monde, touchant de 60 à 90 % des enfants scolarisés, et près de 100 % des adultes. Le xylitol, un édulcorant naturel de structure cristalline, est utilisé depuis la fin des années 1970 en Finlande et dans les autres pays nordiques pour promouvoir la prévention des caries. Lors de sa consommation, il est ingéré par les bactéries cariogènes buccales mais ne peut être transformé en acide, il est donc non-cariogène. Le blocage du métabolisme bactérien succédant à son ingestion lui conférerait, de plus, des propriétés anti-cariogéniques actives par inhibition des caractères de virulence et de la prolifération bactérienne. Ce travail consiste en une analyse critique la littérature disponible, afin de comprendre les différents mécanismes à l'origine de ces propriétés, et de préciser le champs d'action du xylitol ainsi que son intérêt véritable en santé publique dans la prévention de la carie.

Afin de réduire son incidence, plusieurs mesures de prévention ont été mise en oeuvre dont l'utilisation du fluor est la principale représentante. Depuis quelques années, les recherches se tournent vers d'autres produits dont le xylitol fait partie. Cet alcool de sucre est introduit dans de nombreux produits de la vie quotidienne, notamment les chewing-gums et certains produits d'hygiène buccodentaire. Cette scoping review a pour but de faire état des preuves disponibles concernant l'efficacité de l'utilisation du xylitol en odontologie préventive mais, aussi de faire le point sur l'information des chirurgiens-dentistes et des consommateurs. Ce travail de recherche sous forme de scoping review a pour but de faire le point sur la littérature scientifique disponible ces dix dernières années traitant de l'utilisation du xylitol en odontologie préventive. En incluant les divers travaux de synthèse et les articles originaux traitant de l'efficacité du xylitol mais aussi des études moins standardisées s'intéressant aux connaissances et aux comportements des professionnels et des utilisateurs. Cette thèse tente d'offrir une vision étendue des textes scientifiques disponibles. L'objectif de ce travail est de savoir s'il peut y avoir un consensus ou non sur l'utilisation préventive du xylitol et quelles sont les pistes nécessitant des recherches approfondies.

Les lactobacilles en particulier transforment le glucose en acide acétique, en éthanol et surtout en acide lactique. Un geste de prévention bucco-dentaire consiste à substituer ce glucose cariogène par des produits considérés à faible potentiel acidogène tel que le xylitol. Le xylitol est un polyol en C5 auquel ont été attribuées depuis 30 ans de nombreuses propriétés : non acidogène, anticariogène, réducteur de plaque, reminéralisant et inhibiteur de la croissance bactérienne.Nous étudions in vitro le comportement de 20 souches de lactobacilles en présence de xylitol. Nous montrons qu'en fait les lactobacilles peuvent métaboliser le xylitol comme le montre la chute de pH consécutive à la culture de ces bactéries sur un milieu riche en xylitol. Nous expliquons ce fait par une sélection de souches résistantes au xylitol chez lesquelles a eu lieu une dé-répression enzymatique. Cette adaptation est fonction de la durée de culture préalable sur un milieu normalement concentré en xylitol ou en glucose. Enfin nous obtenons 100% de réponses positives, y compris chez des souches du groupe I (homofermentaires) décrites comme incapables d'un tel métabolisme. Nous concluons que si le xylitol est un produit aujourd'hui utile dans la prévention dentaire, à terme, l'adaptation des lactobacilles pourrait le rendre inefficace.
