C'est la première maladie chronique chez les enfants et notamment les adolescents. L'éducation thérapeutique fait partie intégrante du traitement et de la stratégie de prise en charge du patient asthmatique. En 2018, la population des 12-18 ans fait partie de la génération "Internet", ils consacrent quotidiennement un temps significatif à la recherche d'informations en ligne. De ce fait, ils sont prédisposés à s'orienter vers ce moyen de communication devenu universel. Actuellement plusieurs sites Internet sont disponibles comme aide à l'éducation thérapeutique des asthmatiques mais sont peu connus et utilisés par les médecins généralistes. Ils peuvent être un bon lien avec les adolescents et une aide précieuse pour personnaliser leur prise en charge. Nous avons réalisé une étude cas-témoin prospective chez des patients âgés de 12 à 18 ans au sein des consultations de pédiatrie de l'hôpital Gonesse. Une première consultation a lieu avec remplissage d'un questionnaire sur la connaissance de leur pathologie et sur leur qualité de vie. Nous proposons à ce moment-là plusieurs sites Internet sur lesquels ils pourront compléter l'éducation dispensée par le médecin pendant la consultation. Une seconde consultation sera réalisée à 3 mois avec un questionnaire post test sur la qualité de vie et la connaissance de la maladie. Nous avons recueilli au total 43 questionnaires avec 23 patients témoins et 20 patients tests. Notre étude ne montre pas d'impact significatif du soutien d'Internet comme aide à l'éducation thérapeutique des adolescents asthmatiques notamment sur le contrôle de la maladie. En revanche, on note un impact réellement positif sur le ressenti de l'expérience pour ces adolescents avec 85% déclarant que les informations recueillies leur ont été utiles pour mieux comprendre et appréhender leur asthme. Il semble ainsi intéressant de proposer des visites sur les sites médicaux bien documentés aux adolescents afin de les sensibiliser à leur pathologie et essayer de trouver une autre forme d'information encore plus adaptée à leur mode de vie notamment des applications mobiles.

Malheureusement, d'après plusieurs études, celui-ci est souvent incomplet. L'objectif était de mettre en évidence ces obstacles empêchant les bonnes pratiques de suivi d'un patient asthmatique par un médecin généraliste Méthode : il s'agit d'une étude qualitative par théorisation ancrée menée auprès de médecins généralistes des Côtes-d'Armor par entretiens semi-dirigés Résultats : la saturation des données fut obtenue au bout de 10 entretiens auprès de médecins avec des profils différents. Parmi les multiples obstacles, étaient retenus surtout le manque de temps, présent à tous les stades du suivi, le défaut d'éducation thérapeutique et la difficulté d'acceptation des patients. L'asthme était souvent banalisée par les patients et médecins et était fréquemment un motif de consultation secondaire, évoqué rapidement en fin de consultation. Un défaut de formation était évoqués avec d'anciens schémas décisionnels et de trop nombreuses spécialités médicamenteuses. La pollution atmosphérique n'avait été que peu mentionnée, dû à la patientèle rurale et semi-rurale. Discussion : les techniques de prise médicamenteuse étaient souvent mal comprises de leurs patients et beaucoup se reposaient sur les pharmaciens pour l'enseignement. Une mauvaise éducation initiale compliquait la suite de l'ensemble de la prise en charge. Chez aucun des médecins, les plans d'action écrits n'étaient utilisés chez l'adulte. Les services ambulatoires d'éducation thérapeutique étaient soit absents de la région soit méconnus des médecins.

Pour essayer de comprendre certaines lacunes dans la prise en charge du patient asthmatique, cette thèse s'appuie sur certains travaux et études, dont l'étude URCAM réalisée en juin 2001, auprès d'un échantillon de la population francilienne de 10 à 44 ans. Cette étude permet de montrer certains problèmes dans la prise en charge de cette maladie respiratoire, comme : - une attitude face à la crise d'asthme parfois inadaptée ; - la réalisation des examens complémentaires qui parfois fait défaut ; - l'importance d'un traitement médicamenteux couplé à une bonne éducation du patient asthmatique ; - une prise en charge adaptée des facteurs environnementaux, en particulier le tabac. En conclusion, ce travail montre qu'un nombre important de patients asthmatiques restent gênés dans leur quotidien malgré un traitement thérapeutique adapté, d'où l'importance de l'éducation de l'asthmatique et de la formation médicale continue du médecin.
