Book excerpt: Les recommandations actuelles conseillent la mise en place des moyens de prévention bucco·dentaire de manière précoce, -avant 3 ans-, pour une efficacité optimale. Afin de faire passer, de manière adaptée et efficace, les informations nécessaires aux futurs et jeunes parents, elles préconisent, aussi, l'implication de tous les intervenants en relation avec la périnatalité. Pourtant, à l'heure actuelle aucune campagne de prévention ne concerne la petite enfance, et les professionnels au contact des intéressés sont peu ou pas formés. Cette thèse montre, dans un premier temps, la place stratégique que pourrait avoir le maïeuticien dans la mise en place de gestes simples et des recommandations de prévention bucco-dentaire pour le tout petit, au travers des informations qu'il pourrait donner aux futurs parents. L'enquête réalisée au cours de ce travail auprès de 60 maïeuticiens montre leur manque de connaissances sur le sujet, malgré l'intérêt qu'ils lui portent. La dernière partie de la thèse propose une synthèse des notions fondamentales nécessaires à la compréhension de la carie de la petite enfance et de ses conséquences, qui pourrait servir de base à l'élaboration d'un enseignement informatif à l'attention des maïeuticiens. Cet enseignement leur donnerait la possibilité de transmettre les recommandations nécessaires, afin de prendre soin du capital santé que sont les dents temporaires des enfants.

Book excerpt: La carie de la petite enfance est un problème de santé publique majeur et l'une des pathologies les plus rencontrées chez l'enfant. C'est une maladie infantile plurifactorielle, dont certains pouvant être influés par les parents : la consommation sucrée, l'allaitement, l'hygiène bucco-dentaire, la prise de fluor. Ce sont des facteurs modifiables, et qui pourraient évoluer via une intervention d'éducation centrée sur ces sujets. Le but de cette étude est d'évaluer l'impact d'une action de prévention bucco-dentaire auprès de la femme enceinte pour son futur enfant. Il est également de montrer l'existence ou non d'une corrélation entre le niveau bucco-dentaire de l'enfant et de la mère, ainsi qu'estimer l'impact du niveau socio-économique de la mère. Pour cela un examen bucco-dentaire de la femme enceinte puis de l'enfant sera réalisé, associé à un questionnaire à remplir pour juger les connaissances bucco-dentaires et renseigner le niveau socio-économique, et un prélèvement salivaire. L'analyse statistique sera comparative entre les 2 groupes randomisés (1 avec prévention et 1 sans prévention), et descriptive au sein de chaque groupe.

The Environmental Protection Agency's decision in the summer of 2001 to delay implementing a new, more stringent standard for the maximum allowable level for arsenic in drinking water generated a great deal of criticism and controversy. Ultimately at issue were newer data on arsenic beyond those that had been examined in a 1999 National Research Council report. EPA asked the National Research Council for an evaluation of the new data available. The committee's analyses and conclusions are presented in Arsenic in Drinking Water: 2001 Update. New epidemiological studies are critically evaluated, as are new experimental data that provide information on how and at what level arsenic in drinking water can lead to cancer. The report's findings are consistent with those of the 1999 report that found high risks of cancer at the previous federal standard of 50 parts per billion. In fact, the new report concludes that men and women who consume water containing 3 parts per billion of arsenic daily have about a 1 in 1,000 increased risk of developing bladder or lung cancer during their lifetime.

Rapid advancements in medical technologies, breakthroughs in understanding the genetic underpinnings of health and ill health, improvements in the effectiveness and variety of pharmaceuticals, and other developments in biomedical research have helped develop cures for many illnesses and improve the lives of those with chronic diseases. By itself, however, biomedical research cannot address the most significant challenges to improving public health. Approximately half of all causes of mortality in the United States are linked to social and behavioral factors such as smoking, diet, alcohol use, sedentary lifestyle, and accidents. Yet less than five percent of the money spent annually on U.S. health care is devoted to reducing the risks of these preventable conditions.
