Al tener la sensación de que el tiempo no nos alcanza y de que esa falta de tiempo nos agobia, lo más perjudicial es tender a la "acción sin sentido", es decir, creer que haciendo más y más cosas estamos aprovechando mejor nuestro tiempo. Esta dinámica sólo genera agotamiento y frustración, ya que consumiremos todas nuestras energías en tratar de alcanzar un destino que no hemos definido. Claramente ese esfuerzo resulta finalmente inútil, ya que el estar o permanecer muy ocupado, no es ningún referente de que se estén realizando acciones útiles. Alguien muy ocupado habitualmente termina siendo poco productivo. En el libro se plantea la interrogante ¿Por qué se nos genera la necesidad de gestionar algo que no existe?, y se presenta la idea de que el Tiempo no existe (de la forma en que habitualmente lo entendemos), para desde esa premisa abordar la "acción con sentido" y emprender la ruta de la productividad con un criterio práctico de utilidad. ¡Hágalo simple, hágalo útil!. Es complejo hacer el intento de gestionar algo que no entendemos correctamente, ya sea porque tenemos un conocimiento parcial de ello o porque tenemos supuestos que no son del todo correctos. En este libro se expone la relevancia del "ahora" y de la utilidad de nuestras acciones. Se explica la importancia de tener claramente establecido un propósito, ya que sin un propósito definido, no hay acción que resulte útil, por tanto no hay productividad posible de obtener y con cualquier cosa que hagamos que no esté correctamente enfocada con nuestro propósito estaremos desperdiciando nuestro tiempo. Se entregan insumos para identificar, en uno mismo y en los demás, las fugas de tiempo, entender de dónde vienen, saber si estoy haciendo algo para detenerlas y de manera consciente actuar sobre ellas. Se expone además el concepto de que el tiempo habita en cada uno de nosotros, por lo que resulta necesario al "gestionar el tiempo", que sepamos gestionar adecuadamente nuestro bienestar físico y sicológico. En el libro se detallan algunas pautas para enfrentar esta problemática y apuntar a la eficiencia y la productividad, sin ser atormentados por el látigo del tiempo; sin embargo las mejores recetas resultan cuando cada uno las enriquece con su propia experiencia, cuando se aprende a identificar cada uno de los insumos, se entiende cómo aportan y se establece una forma propia de utilizarlos para obtener el producto final.

At the same time, however, the United States is facing the vexing challenge of a seemingly uncontrolled rise in the cost of health care. Total medical expenditures are rapidly approaching 20 percent of the gross domestic product and are crowding out other priorities of national importance. The use of increasingly expensive prescription drugs is a significant part of this problem, making the cost of biopharmaceuticals a serious national concern with broad political implications. Especially with the highly visible and very large price increases for prescription drugs that have occurred in recent years, finding a way to make prescription medicinesâ€"and health care at largeâ€"more affordable for everyone has become a socioeconomic imperative. Affordability is a complex function of factors, including not just the prices of the drugs themselves, but also the details of an individual's insurance coverage and the number of medical conditions that an individual or family confronts. Therefore, any solution to the affordability issue will require considering all of these factors together. The current high and increasing costs of prescription drugsâ€"coupled with the broader trends in overall health care costsâ€"is unsustainable to society as a whole. Making Medicines Affordable examines patient access to affordable and effective therapies, with emphasis on drug pricing, inflation in the cost of drugs, and insurance design. This report explores structural and policy factors influencing drug pricing, drug access programs, the emerging role of comparative effectiveness assessments in payment policies, changing finances of medical practice with regard to drug costs and reimbursement, and measures to prevent drug shortages and foster continued innovation in drug development.
