La Classification SAC est utilisée depuis 2003 dans le programme pédagogique de l'ITI Le présent volume est un outil de référence indispensable à tous les praticiens pour aider à la prise de décision adaptée dans chaque cas à traiter Grâce à la clarté des descriptions et de la Classification, les praticiens peuvent évaluer le niveau de complexité et de risque de chaque cas et les compétences nécessaires pour réussir le traitement. Le présent volume est non seulement une aide à la planification du traitement pour les praticiens, mais aussi un outil pour les enseignants désireux d1élaborer des programmes de formation à l'implantologie par niveaux croissants de difficulté."

Book excerpt: L'implantologie fait figure de véritable révolution dans la pratique de la dentisterie depuis maintenant plus de trente ans. En effet, l'implant dentaire a prouvé qu'il était une alternative de choix aux prothèses conventionnelles, qu'elles soient amovibles ou dento-portées. Cependant, comme pour une dent naturelle, l'implant dentaire n'est pas exempt de complications. On retrouve plusieurs types de maladies péri-implantaires. Absentes de la classification d'Armitage (parue en 1999), il faudra attendre la récente classification de Chicago (en novembre 2017) pour qu'elles soient reconnues et classifiées officiellement. Dès lors, trois grandes définitions ont été proposées : la santé péri-implantaire, la mucosite et la péri-implantite. Chacune d'elles ayant des critères diagnostiques bien précis. La durée de vie d'un implant dépend en grande partie de l'hygiène bucco-dentaire et donc de la capacité qu'aura le patient à désorganiser le biofilm bactérien. La prévention des complications péri-implantaires est articulée autour de la gestion des facteurs de risques généraux et locaux du patient mais aussi de la maintenance individuelle et professionnelle. Bien qu'il n'existe pas de protocole « miracle » garantissant la parfaite santé des tissus périimplantaires, nous essaierons grâce aux données recueillies dans la littérature de proposer un protocole de soutien « type » qui pourra être systématisé à chaque visite de maintenance. Ce dernier comporte 4 objectifs majeurs qui doivent impérativement être remplis par le professionnel de santé bucco-dentaire : informer, contrôler, motiver et assainir. Cette étude bibliographique permet de constater que la prise en charge la plus efficace des maladies péri-implantaires reste la prévention qui impose d'informer le patient sur les risques, de le motiver et de le former à une hygiène bucco-dentaire efficace.

Mais aujourd'hui de nouvelles interrogations se posent quant au devenir sur le long terme des implants. Cette étude s'intéresse à la survie implantaire à long terme (plus de 10 ans) des implants posés au sein du service d'implantologie de l'unité fonctionnelle de chirurgie orale de Clermont-Ferrand. L'analyse se base sur les critères de classification de la HSDI (Health Scale for Dental Implant) et prend en compte des facteurs secondaires liés au patient, à l'implant et à la chirurgie. Le taux d'échec constaté est d'environ 35,3% et plusieurs facteurs se révèlent être statistiquement significatifs selon les analyses de Kaplan-Meier.
