Des critères faciaux, dentaires et parodontaux précis définissent l'idéal vers lequel celui-ci devrait tendre pour une intégration esthétique optimale. Une ligne du sourire très haute, dévoilant plus de 2 mm de gencive, est jugée inesthétique par les patients et les praticiens ; on parle familièrement de sourire gingival. La chirurgie de replacement labial est une intervention minimalement invasive visant à réduire l'élévation de la lèvre supérieure lors du sourire. Elle est décrite en 1973 et adaptée en 2006 pour une réalisation au cabinet dentaire. Les études réalisées permettent d'estimer la diminution moyenne d'exposition gingivale à 3,4 mm 6 mois après l'intervention. Cette chirurgie constitue une option thérapeutique simple, présentant peu de complications et des suites opératoires réduites par rapport à une chirurgie d'impaction maxillaire. L'identification des étiologies en cause et leur association aux bonnes options thérapeutiques permet une prise en charge rationnelle et un résultat plus prédictible. La réalisation d'un arbre décisionnel guide le chirurgien-dentiste dans sa démarche diagnostique et lui permet de systématiser sa prise en charge.

Enfin, nous discuterons de cette méthode (sa fiabilité, ses résultats à long terme, la satisfaction des patiences, etc.).
