Book excerpt: As global populations age, governments around the world are investigating how to fund long-term care (LTC) in an equitable and sustainable manner. The research reported here has three objectives: (i) to identify and classify middle-income countries (MICs) and high-income countries (HICs) that have established LTC for older populations; (ii) to describe the financing features and undertake a detailed assessment of the public LTC programmes in these countries; and (iii) to identify and discuss the benefits, disadvantages and challenges of the different public LTC financing strategies, based on the experiences of high-income countries and on observations of the reviewed countries. The public LTC financing system of 13 countries is reviewed: five HICs (Australia, Japan, the Netherlands (Kingdom of the), Singapore and Uruguay), and eight MICs (China, Costa Rica, India, Indonesia, Malaysia, Serbia, South Africa and Thailand). Although information on LTC expenditure is not consistently reported or available for all countries, the 13 reviewed countries vary considerably in terms of their national income, total spending on health and public share of health care spending. This report concludes with some deliberations and lessons learned on financing options for LTC, specifically for low- and middle-income countries. Overall, the report offers valuable insights into how policy-makers can design effective and sustainable public LTC financing systems, ensuring that individuals and their families receive the necessary support and assistance to lead dignified lives as they age.

The book compares key features of long-term care insurance (LTCI) schemes in 15 pilot cities and evaluates the sustainability of various financing models adopted by the cities in the LTCI schemes. The book uses an interpretive case study approach to give an in-depth look into the LTC models in three pilot cities – Qingdao, Nantong, and Shanghai. The three cities represent three different models of financing and delivering LTC. To assess how effective the LTC models in these three cities are, the book uses five criteria, including utilization of medical resources, cost, equity, quality of care and sustainability. Also, the authors discuss how the financing and delivery of LTC can be improved in China, the impact of the 2019 coronavirus (COVID-19) pandemic on older adults in need of LTC in the country and the implications of China’s LTCI reform for other countries. Book excerpt: Over the last two decades, many changes have happened to the social welfare policies of various industrial countries. Citizens have seen their pensions, unemployment benefits, and general healthcare policies shrink as “belt tightening” measures are enforced. But in contrast, long-term care has seen a general growth in public financing, an expansion of beneficiaries, and, more generally, an attempt to define larger social responsibilities and related social rights. The aim of this book is to describe and interpret the changes introduced in long-term care policies in Western Europe. The volume argues that recent reforms have brought about an increasing convergence in LTC policies. Most of the new programs have developed a new general approach to long-term care, based on a better integration of social care and health care. The book explores increasing public support given to family care work (in the past, the family would take care of the elderly or infirm) and increasing growth and recognition of a extended social care market (by which care has shifted from a moral obligation based on family reciprocity to a paid, professional activity). A new social care arrangement has therefore been developing in Western countries, based on a new mix of family obligations, market provision, and public support. In order to understand such changes, this analysis will take into account the social and economical impact of these reforms.
