Contexte : Les troubles de l’oralité dans l’autisme sont fréquents et peuvent entraîner des troubles alimentaires, entraver la mise en place des conduites d’instrumentation et le développement du langage. Méthodes : Pour cela, nous avons choisi une méthodologie théorico-clinique. Nous abordons le thème de l’oralité dans le développement psychoaffectif de l’enfant. Puis nous développons les troubles sensori-perceptuels rencontrés dans l’autisme et le modèle neurosensoriel en nous centrant sur les troubles liés à la sphère orale intervenant dans le développement de l’enfant avec autisme. Ensuite, nous tentons de replacer dans une approche psychodynamique l’oralité dans le développement psychocorporel de l’enfant autiste. Nous abordons les différentes approches thérapeutiques permettant l’investissement de la zone orale dans ce trouble. Nous évoquons les troubles alimentaires qui découlent de cette problématique. Enfin, nous étayons notre propos avec quatre cas cliniques incluant une observation des repas. Conclusion : Au regard des observations cliniques et de la littérature, des différences dans la nature et l’intensité des troubles neurosensoriels au niveau de la sphère orale semblent apparaître. Ils engendrent des comportements en lien avec les troubles autistiques. Néanmoins, ils n’expliquent pas l’ensemble des phénomènes observés, et se retrouvent étroitement liés au développement psychocorporel, émotionnel et relationnel de l’enfant dont les premiers fondements s’établissent autour de l’expérience orale du nourrissage.

Book excerpt: Les troubles de l’alimentation sont très présents au sein de la population des enfants souffrant d’un trouble du spectre autistique (TSA) : 56 à 87% (Manikam et Perman, 2000). Ces troubles proviennent d’un défaut de traitement des stimuli sensoriels -hypo ou hypersensibilité- et/ou d’un retard praxique oro-facial. L’oralité alimentaire et l’oralité verbale étant intimement liées, les capacités verbales sont souvent amoindries au sein de cette population. L’objectif de cette étude était de démontrer l’efficacité d’une rééducation gnoso-praxique de la sphère oro-faciale sur les troubles de l’oralité alimentaire et de l’oralité verbale d’enfants TSA. Nous avons réalisé une étude de trois cas comportant une phase de pré-test, un entraînement et une phase de post-test. Les sujets avaient entre 3 ans et 4 ans 4 mois, étaient diagnostiqués TSA et souffraient de troubles de l’oralité. Différents outils d’évaluation ont été utilisés : une grille d’observation d’un goûter, la passation d’épreuves de langage et de communication, ainsi qu’une épreuve des praxies bucco-faciales issues de la batterie Evalo BB, un entretien semi-dirigé avec les parents et un questionnaire aux professionnels entourant l’enfant. Les enfants ont bénéficié de sept séances de prise en charge gnoso-praxique : sollicitations multi-sensorielles et mouvements volontaires et dirigés des organes bucco-faciaux. Les évaluations ont montré que l'entraînement gnoso-praxique a eu des répercussions positives sur les difficultés alimentaires de deux enfants de notre étude. Il leur a permis d’investir leur zone oro-faciale, de se familiariser avec les différentes caractéristiques des aliments et d’appréhender les repas de manière plus positive. Cependant, peu de changements ont été constatés en ce qui concerne l’oralité verbale. Cette étude démontre que la rééducation gnoso-praxique de la zone oro-faciale est adaptée cliniquement aux enfants TSA et qu’elle permet d’amoindrir leurs troubles alimentaires. Il serait intéressant de reproduire ce protocole de rééducation sur une durée de 6 mois minimum, en augmentant le nombre de sujets et en utilisant des outils d’évaluation plus objectifs
