Book excerpt: Drawing on anthropology, historical sociology and social-epidemiology, this multidisciplinary book investigates how pharmaceuticals are produced, distributed, prescribed, (and) consumed, and regulated in order to construct a comprehensive understanding of the issues that drive (medicine) pharmaceutical markets in the Global South today. Based on primary research conducted in Benin and Ghana, and additional data collected in Cambodia and the Ivory Coast, this volume uses artemisinin-based combination therapies (ACTs) against malaria as a central case study. It highlights the influence of the countries colonial and post-colonial history on their models for state regulation, production, and distribution, explores the determining role transnational actors as well as industries from the North but also and increasingly from the South play in influencing local pharmaceutical markets and looks at the behaviour of health care professionals and individuals. Stepping back, the authors then unpick the pharmaceuticalization process and the multiple regulations at stake by looking at the workings of, and linkages between, (biomedical health) pharmaceutical systems, (representatives of companies) industries, actors in private distribution, and consumer practices. Providing a thorough comparative analysis of the advantages and disadvantages of different pharmaceutical systems, it is an important contribution to the literature on pharmaceutalization and the governance of medication. It is of interest to students, researchers and policy-makers interested in medical anthropology, the sociology of health and illness, global health, healthcare management and pharmacy. The explosion of the HIV/AIDS pandemic, the persistence of malaria as a major killer, and the resurgence of diseases like tuberculosis which were previously under control, have brought about changes in the health system, with implications for its governance, especially in view of the diminished capacity of the public health facilities to cope with a complex range of expanded needs. Government responsibilities and objectives in the health sector have been redefined, with private sector entities (both for profit and not-for profit) playing an increasingly visible role in health care provisions. The reasons for collaborative patterns vary, but chronic under-funding of publicly financed health services is often an important factor. Processes of decentralisation and health sector reforms have had mixed effects on health care system performance; while private health insurance markets and private clinics are pointers to a growing stratification of the health market, in line with the intensified income and social differentiation that has occurred over the last two decades.These developments call for health sector reforms.

Dans ce paysage très évolutif, le gouvernement français, à l'occasion du Comité interministériel de la recherche scientifique et technique du 15 juillet 1998, a retenu la proposition du Ministre de l'Education nationale, de la Recherche et de la Technologie de confier à l'Académie des sciences le soin d'établir un rapport sur l'état de la science et de la technologie en France. Ce rapport est la synthèse de 11 rapports thématiques qui ont été remis au ministre le 27 juillet 2000. Le rapprochement des disciplines et des techniques scientifiques dans l'abord des problèmes et dans l'étude des systèmes complexes devrait inciter à ce qu'un accent plus net soit mis sur des projets pluridisciplinaires développés entre plusieurs organismes de recherches ainsi qu'avec des universités et des entreprises. Cela devrait impliquer aussi une vision nouvelle dans les formations de l'enseignement supérieur, dans la politique des grands équipements, assortie d'une facilitation de la mobilité des chercheurs, tant au sein de l'hexagone qu'à l'extérieur. La France doit affirmer davantage sa participation aux grands " programmes-cadres " de l'Union européenne, qui ont déjà contribué au décloisonnement des équipes, même si l'Académie reconnaît l'urgence d'une simplification des procédures et la nécessité d'une intervention plus directe des scientifiques dans l'établissement et le suivi de ces programmes. Par ailleurs, il apparaît nécessaire que la communauté scientifique française prenne mieux en compte les enjeux sociétaux et les problèmes éthiques dans un contexte d'ensemble où l'image de la science perd parfois de sa force. Au-delà des propositions liées à chacune des disciplines traitées, les auteurs formulent des recommandations visant à adapter le système français de recherche/développement et d'enseignement aux nouvelles données scientifiques et économiques.
