Book excerpt: Cette recherche a gravité autour des soins offerts aux familles de patients atteints de troubles mentaux et qui sont hospitalisés en milieu de psychiatrie légale. Le but de cette recherche doctorale était d'étudier le phénomène de la violence familiale perpétrée par un proche atteint de troubles mentaux et de mettre en lumière les dynamiques qui existent entre les acteurs impliqués (familles, milieux hospitaliers et organismes de soutien). Selon les écrits recensés, les familles vivant avec un proche atteint d'un trouble mental sont bien souvent confrontées à une problématique de violence. En effet, près de la moitié de ces familles en sont victimes. On observe dans la pratique clinique et en recherche la difficulté des familles à devoir conjuguer la violence et la maladie. Il ressort des écrits que ces familles doivent faire intervenir les policiers et effectuer des démarches juridiques afin de faire hospitaliser le proche contre son gré. Cette réalité entraîne une animosité chez le proche à l'endroit de sa famille, et peut exacerber les perceptions paranoïdes ou persécutrices. Pareille interaction entre la psychiatrie et le système judiciaire est par contre délétère pour la relation familiale. Ce projet doctoral avait pour but d'étudier l'expérience vécue des membres de ces familles, qui sont à la fois soignants et victimes de violence. Nous voulions également explorer les impacts de la violence à l'endroit des membres de la famille ainsi que les stratégies utilisées afin de la prévenir. La théorie du gouvernement des familles de Donzelot (1977/2005) a été préconisée comme cadre théorique. Le concept de gouvernementalité englobe les différents mécanismes de pouvoir et de contrôle qui s'exercent à l'endroit de la famille. Sur le plan méthodologique, le devis est de type qualitatif et la théorisation ancrée a été utilisée comme stratégie de recherche. Cette méthode de recherche a pour objectif de mettre en lumière les processus, les interactions et le vécu des participants (Corbin et Strauss, 2014). Lors de la collecte de données, 14 participants ayant été victimes de violence perpétrée par un proche atteint de troubles mentaux sévères ont été recrutés. Les participants provenaient de dix familles différentes à travers la grande région de Montréal, et ont été rencontrés dans le cadre d'entretiens semi-structurés en individuel, en couple ou en groupe. Des notes d'observation et des mémos analytiques ont été rédigés, et ont aussi fait partie du matériel analysé. Cinq thèmes ont émergé de l'analyse qualitative des données : 1) le dispositif médico-légal ; 2) l'expérience de la violence ; 3) la prise en charge du proche par la famille ; 4) les mécanismes d'exclusion et de stigmatisation ; et 5) souffrances et résilience. Les principaux résultats de notre recherche indiquent qu'un gouvernement de la famille s'effectue par l'entremise de certains mécanismes, dont l'instrumentalisation du rôle de la famille et le transfert du mandat de soin à cette dernière. Nous avons pu observer qu'un double mandat (soin et contrôle) est imposé à la famille. L'existence de ce double mandat engendre une ambivalence affective chez les familles et est source de tensions internes. Nous avons aussi observé la présence de barrières à l'inclusion des familles dans la trajectoire de soins, soit entre autres la confidentialité, le vocabulaire psychiatrique hermétique et la stigmatisation de la famille en présence de la violence. En résumé, cette recherche s'avère pertinente pour la discipline et la pratique infirmières puisqu'elle permet d'éclairer les interventions cliniques et les politiques de santé par rapport aux soins à la famille. Elle est aussi pertinente afin de pouvoir offrir des soins de qualité adaptés à une situation familiale des plus complexes. Enfin, grâce à cette recherche, nous avons tenté de donner une voix aux membres de familles, sur qui pèse un lourd fardeau.

The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
