En 2005 est mis en place le Plan Périnatalité de 2005-2007, et la Haute Autorité de Santé publie des recommandations professionnelles à propos de l'Entretien Prénatal Précoce dans le cadre de la Préparation à la Naissance et à la Parentalité. OBJECTIFS: Notre objectif général était d'optimiser la réalisation des entretiens prénataux dans la maternité de la clinique du Diaconat Fonderie de Mulhouse, en mettant en avant différents axes d'amélioration. MÉTHODES : Nous avons effectué un audit, avec une distribution de questionnaires : un à destination des patientes, et un à destination des professionnels. L'étude descriptive, quantitative, observationnelle et prospective a été réalisée dans la maternité de la Clinique du Diaconat Fonderie, à Mulhouse. RÉSULTATS : 41 patientes et 3 sages-femmes ont été recrutées. L'entretien a été réalisé en moyenne au milieu du 5ème mois de grossesse. Ces dernières ont attribué plusieurs notes sur dix : 9,84/10 en moyenne pour la qualité et le nombre d'informations reçues, et 9,74/10 concernant leur satisfaction globale. Un tiers des patientes a été orienté vers un autre professionnel de santé. DISCUSSION :Le taux de participation à l'entretien était estimé à 24,1% en 2018. Nous avons donc réalisé une plaquette d'information à destination des patientes pour les informer de l'existence de l'EPP. Les recommandations de la HAS ont été globalement correctement respectées. Cependant, l'EPP a été réalisé tardivement dans la grossesse, après le premier trimestre pour la totalité des patientes interrogées ; et une des professionnelles n'avait pas connaissance des recommandations de la HAS.

Matériel et méthode: Une étude observationnelle descriptive à viser étiologique a été réalisée. Un questionnaire a été diffusé aux sages-femmes réalisant des suivis de grossesse et déposé sur le lieu d'exercice à un échantillon restreint de médecins généralistes et gynécologues obstétriciens d'Indre et Loire. Résultats : L'analyse du questionnaire n'a révélé aucun frein de manière spécifique à la proposition et/ou, à la réalisation de I'EPP si ce n'est le manque de temps à sa réalisation et l'absence de formation des professionnels de santé à cet entretien. Par ailleurs, les professionnels effectuant cette consultation la font majoritairement à leur propre patientèle. Notons, que la synthèse de cet entretien n'est majoritairement pas partagée avec d'autres professionnels de santé. Conclusions : La transmission entre les professionnels de santé est à améliorer ce qui contribuerait certainement à améliorer le taux de proposition et réalisation de cet entretien.

Book excerpt: A la Maison Santé Pluridisciplinaire de Cluny, l’Entretien Prénatal Précoce est proposé à toutes les femmes enceintes depuis 2013. Après un an de mise en place, l’évaluation de ce projet de santé territorial s’imposait. Nous avons utilisé une approche quantitative sur les données recueillies à chaque entretien et une approche qualitative avec dix interviews rétrospectifs, semi-directifs. 127 entretiens ont été réalisés en 2013. 16,9% des femmes ont été recrutées par un médecin généraliste ou gynécologue. 1/3 des femmes enceintes suivies par un médecin généraliste de la Maison de Santé ont bénéficié d’un entretien. Les femmes avaient en moyenne 30 ans+/-4,1, étaient socialement privilégiées et 58% d’entre-elles étaient primipares. L’entretien a majoritairement été tenu au 2ème trimestre. Si dans 49,6% des cas il n’a révélé aucun problème, 21,3% des entretiens ont décelé un problème psychologique, lié à l’angoisse de l’accouchement ou de la grossesse. Le poids moyen des enfants nés de femmes fumeuses (3116g) étaient significativement plus faible que ceux nés de mères non fumeuses (3376g) (p=0,02). Le repérage des conduites addictives a été insuffisant. Les femmes interrogées ont défini l’entretien comme une aide à la parentalité et le réitéreraient. L’entretien tel qu’il est organisé ne répond pas aux objectifs. Le médecin généraliste, formé à la prise en charge globale, serait le plus apte à appréhender cet entretien mais les freins à sa réalisation seraient à évaluer. La coopération interprofessionnelle doit se développer pour établir des projets de santé pertinents. L’entretien postnatal précoce est une perspective à envisager sollicitée par les femmes elles-mêmes.

Book excerpt: L'objectif principal de notre étude était de déterminer la proportion de femmes qui avaient bénéficié de l'entretien prénatal précoce, et d'identifier les caractéristiques psycho-sociales et obstétricales de ces femmes. L'objectif secondaire était d'étudier les modalités de réalisation de l'EPP. Notre étude descriptive rétrospective portait sur les femmes présentes à la maternité du CHU de Saint Etienne au mois de juin 2017. Un questionnaire leur était présenté, avec des questions générales les concernant, des questions sur leur grossesse, leur accouchement, et le déroulement de leur entretien prénatal précoce. Seulement 30% des femmes enceintes ont bénéficié de l'entretien prénatal précoce. Les primipares avaient significativement plus participé à cet entretien que les multipares, tout comme les femmes de catégorie socio-professionnelle élevée y participaient davantage. Cependant, les femmes qui déclarent avoir eu des difficultés durant leur grossesse ne semblent pas en bénéficier davantage. Notre étude montre que l'entretien prénatal précoce ne semble pas remplir son objectif d'aide aux femmes enceintes vulnérables. Bien que la plupart des entretiens soient globalement respectueux des recommandations de la HAS, cet outil de prévention n'est pas systématiquement proposé à toutes les femmes enceintes. Les femmes présentant des facteurs de vulnérabilité devraient être les cibles de cet entretien.
