Book excerpt: Les patients avec une douleur oro-faciale chronique peuvent être rencontrés au sein d'un cabinet dentaire libéral. Le chirurgien-dentiste a un rôle important à jouer, notamment parce qu'il est souvent le premier à être consulté. La prise en charge de ces patients est difficile et complexe ; elle bouscule le praticien dans sa clinique car elle l'oblige à changer de paradigme, à abandonner l'approche biomédicale. Si le chirurgien-dentiste établit un diagnostic précoce d'exclusion d'une pathologie bucco-dentaire, il n'a pas toujours conscience que l'accueil bienveillant de la plainte douloureuse est primordial pour le futur. Le praticien est souvent désarmé de ne pouvoir proposer une solution médicotechnique efficace et a souvent l'impression d'être inutile auprès de ces patients. Ces douleurs nécessitent une prise en charge biopsychosociale, quelque fois associée à un travail psychothérapeutique. Le praticien doit donc résister à toute envie de réaliser un acte technique invasif pour privilégier l'écoute du patient et de son vécu douloureux. Reconnaitre la réalité de cette douleur permet d'instaurer une relation thérapeutique, une relation authentique et de qualité entre patient et praticien qui permettra d'éviter l'errance médicale et toute iatrogénie dans la prise en charge. L'objectif de ce travail est de mettre en évidence comment la relation thérapeutique entre le chirurgien-dentiste et le patient douloureux constitue un premier levier, comment elle conditionne l'acceptation du patient et son engagement dans une prise en charge multidisciplinaire. Le patient renoncera alors à une recherche incessante de solutions médicales pour envisager la douleur sous d'autres prismes : relationnel, social, psychologique et spirituel. Le chirurgien-dentiste a donc sa place dans ce processus en abandonnant sa posture d'expert exclusivement centrée sur le somatique, pour une posture humaniste et d'ouverture. Prendre le temps d'installer une relation de qualité avec le patient atteint de douleurs oro-faciales chroniques est donc une étape fondamentale avant de l'adresser à une Structure Douleur qui poursuivra la prise en charge.

Des techniques thérapeutiques variées, pharmacologiques mais aussi psychosociales, doivent, le plus souvent, être combinées pour aider le patient à maîtriser progressivement sa douleur. Nous nous intéresserons à l’une de ces thérapies psychosociales l’art-thérapie, une pratique de soin fondée sur l’utilisation thérapeutique du potentiel de création artistique. Il s’agit, par le biais de la peinture, du théâtre, de la danse, de l’écriture, de dépasser l’impossibilité d’exprimer par des mots les douleurs, les souffrances, les contradictions, et de les transformer en une production artistique. Dans une première partie, la thèse se concentre sur les caractéristiques de la douleur chronique. La deuxième partie de ce travail est consacré à la définition de l’art-thérapie, ses objectifs, sa méthode, ses mécanismes thérapeutiques et ses médiations. Dans un troisième temps, nous interrogerons des art-thérapeutes et des psychiatres pour tenter d’expliquer comment cette discipline tente d’interagir avec des facteurs étroitement liés au soulagement de la douleur orofaciale participant ainsi à l’amélioration de la qualité de vie du patient.

Book excerpt: The invasive character of a primary cancer is greatly dependent on numerous interactions between tumor cells and their extracellular surroundings. Matricellular receptors are defined as (cell-surface) receptors that bind extracellular matrix (ECM) structural proteins and soluble factors dynamically acting on ECM homeostasis. Matricellular receptors mediate numerous signalings from the extracellular environment to cell nucleus and drive main biological functions that are cell growth, survival and migration. Numerous data from the last decade evidence that matricellular receptors are biosensors that allow to a tumor cell answering to microenvironmental variations, and in this sense they are important contributors to tumor cell malignancy. Matricellular receptors represent thus valuable targets for the development of original anti-cancer strategies. Original reports, bibliographic reviews or hypotheses are welcome to improve the basic knowledge of matricellular receptor properties, their spatio-temporal regulation, the dynamic formation of complex receptors and the impact of such interactions on the invasive properties of tumor cells. Biological, biophysical and pharmacological, as well as in silico contributions will be appreciated.
