L’implantologie permet l’obtention d’un ancrage squelettique absolu indépendamment de la collaboration du patient. Les recherches dans ce domaine ont conduit à la mise au point de différents systèmes allant de l’implant dentaire aux mini-vis; en passant par les implants palatins à usage orthodontique et les mini-plaques. Grâce à la réduction de la taille des implants, les mini-vis permettent d’augmenter le nombre de sites d’ancrage. Leur utilisation est très simple, et leur mise en charge est immédiate. Cependant, il existe des risques d’endommager les structures voisines lors de leur insertion, comme par exemple les racines dentaires. Leurs indications sont nombreuses, mais, l’ancrage par les mini-vis ne doit pas systématiquement se substituer aux ancrages conventionnels il doit permettre de simplifier des situations d’ancrages difficiles ou d’apporter de nouvelles options thérapeutiques, en privilégiant par exemple des traitements sans extraction ou sans chirurgie orthognathique. L’utilisation d’un ancrage squelettique trouvera à l’avenir de plus en plus d’applications en pratique orthodontique. Mais, des recherches futures impliquant des essais randomisés cliniques sont nécessaires pour évaluer les facteurs des taux de succès des mini-vis et ainsi rendre leur utilisation la plus sûre possible, car de telles études font actuellement défaut.

Le développement des plaques d'ancrage en orthodontie, grâce à l'obtention d'un ancrage absolu, a permis d'élargir les possibilités de prise en charge orthodontique. Ces mini-plaques d'ancrage squelettique, notamment utilisées par De Clerck et Sugawara, permettent d'obtenir des mouvements orthodontiques autrefois considérés comme quasi impossibles, repoussant, dans certains cas, les indications de chirurgie orthognatique ou d'avulsions. Bien que l'insertion et la dépose soient plus complexes que celle des minivis, les nombreux avantages liés à leur utilisation, comme une valeur d'ancrage plus importante, en font un allié thérapeutique non négligeable. Les mini-plaques constituent ainsi un système innovant et efficace dans la prise en charge de patients chez lesquels les limites de l'orthodontie classique sont atteintes. Leur utilisation doit toutefois répondre à des indications très précises associées à un rapport bénéfice-risque favorable, chez l'adulte comme chez l'enfant.

La fiabilité, la facilité de mise en œuvre, le faible coût et les nombreuses indications des mini-vis en font le système le plus répandu. Elles peuvent être positionnées entre les racines dentaires ou à l'extérieur du processus alvéolaire. Les limites inhérentes au positionnement inter radiculaire (IR), ont fondé les avantages majeurs des mini-vis extra alvéolaires (EA): 1. Risque réduit de dommage des racines dentaires. 2. Absence d'interférence avec le déplacement des dents. 3. Une plus grande quantité d'os cortical aux points de placement. 4. Taux d'échec plus faible. 5. Ancrage adéquat pour la rétraction de l'arcade dentaire dans son ensemble. 6. Diminution du nombre de TAD nécessaires pour un traitement complexe. 7. Plus grande polyvalence des mouvements dentaires. Parmi les sites EA bien établis il existe la crête infra zygomatique, le plateau mandibulaire vestibulaire et la branche montante de la mandibule. Les mini-vis corticales courtes font également l'objet d'une littérature "développée". Plus récemment, un ancrage zygomatique et symphysaire émerge comme une alternative supplémentaire aux mini-plaques. Avec un taux de réussite global qui dépasse 92 %, les dispositifs d'ancrage temporaire extra-alévolaires sont des auxiliaires bien établis et fiables. Avec des formations complémentaires et une courbe d'apprentissage rapide, le recours aux mini-vis EA élargit considérablement la portée du traitement conservateur des malocclusions sévères et complexes.
