Différentes études ont montré que l'implication maternelle auprès de l'adolescente serait bénéfique pour l'observance et la moindre prise de risques sexuels. Objectif : Evaluer le ressenti et l'attitude du médecin généraliste (MG) face à la présence maternelle lors d'une demande de première contraception d'une adolescente accompagnée de sa mère. Méthode : Étude qualitative par entretiens semi-dirigés retranscrits et analysés par thèmes de 17 MG de juin 2015 à juillet 2015 (Loire, France). Résultats : La mère était fréquemment présente durant toute cette consultation, considérée usuelles par les MG. Selon eux, sa présence facilitait la collecte des antécédents familiaux et libérait la parole de l'adolescente. Certains s'isolaient avec l'adolescente pour recueillir des informations jugées confidentielles. Plusieurs profils maternels ont été établis qui influençaient le déroulement de la consultation. La pilule oestro-progestative était majoritairement prescrite avec le souci d'autonomisation, sauf pour quelques MG sous l'influence de divers jugements ou craintes liés au tabagisme, à l'âge et à la sexualité de l'adolescente. Lors de la mise en route d'une contraception, la majorité des MG parents s'informait sur la sexualité des adolescentes à l'opposé de leurs enfants. Conclusion : La prise en charge de l'adolescente nécessite une approche spécifique de la part du MG. Face à l'adolescente le MG devrait faire fi de ses aprioris et jugements, car il est le médecin de famille de chaque personne la composant. Pour gérer au mieux cette consultation, la formation relationnelle gagnerait à être renforcée afin que le choix contraceptif de l'adolescente soit mieux entendu.

Médecin de famille, le généraliste a une place de premier choix pour aborder la contraception avec l’adolescente. Comment procède-t-il? La réalisation d’une étude qualitative, à l’aide d’une enquête par 10 entretiens semi-dirigés de médecins généralistes de la Somme installés en libéral, a permis de comprendre leur démarche. La prescription initiale de contraception va au-delà des critères médicaux. Même si l’examen médical est une étape incontournable, le médecin fait face à des obstacles. Il doit s’adapter à une relation triangulaire avec la maman révélant un biais de confidentialité et du motif réel de la demande. Il doit orienter son discours selon l’âge de la personne, son contexte socio-culturel, sa motivation et sa rigueur. Le facteur économique est prépondérant mais le défaut de connaissance est aussi important. Il doit délivrer une information idéalement complète sur la contraception, la contraception d’urgence et les IST. Pas simple en pratique, la démarche éducative reste peu utilisée et le manque de temps est réel. Il doit tenter de rendre l’adolescente autonome dans ses choix pour l’amener vers une contraception adaptée à ses besoins. Les autres formes de contraception que la pilule restent peu abordées, les croyances biaisant leur utilisation. Les connaissances du médecin sont fondées sur son expérience professionnelle et son vécu personnel, la formation initiale étant manquante la plupart du temps. Il faut poursuivre les efforts de remboursement, d’information à grande échelle, d’information par le médecin et le développement des connaissances des médecins.

Book excerpt: En France, nous constatons un paradoxe car la large diffusion de la contraception a permis une diminution des grossesses non désirées, mais sans diminuer le nombre d'IVG qui a même tendance à augmenter chez les adolescentes. L'adolescence est une période de fragilité nécessitant une prise en charge particulière. Le but de notre travail est d'étudier la situation actuelle de la contraception chez les adolescentes et la place des médecins généralistes. Nous avons pour cela distribué un questionnaire à choix multiples aux lycéennes de terminale de 5 lycéees du Puy-de-Dôme. Nous avons eu 465 réponses. Notre enquête nous a permis de confirmer l'importante utilisation de la contraception par 77,85% des adolescentes. Dans 80% des cas elles utilisent une pilule oetroprogestative (un tiers seulement sont des pilules remboursées par la Sécurité Sociale). Dans 49,04% des cas cette contraception est prescrite par un gynécologue et dans 39,81% des cas par un médecin généraliste. Lorsqu'elles choisissent un médecin généraliste, les adolescentes recherchent en 1er lieu leur médecin de famille en qui elles ont par ailleurs confiance pour respecter le secret médical dans 92% des cas. En revanche, seule la moitié des élèves ont déjà parlé de contraception avec leur médecin généraliste. Malgré les recommandations de L'ANAES, seules 65% des adolescentes ont déjà été reçues seules par leur médecin. Lorsqu'elles sont reçues seules, le sujet de la contraception est plus souvent abordé. Mais les adolescentes indiquent qu'elles ne souhaitent pas que leur médecin demande à leurs parents de sortir ce qui marque toute l'ambiguïté des adolescentes qui sont encore rassurées par la présence de leurs parents mais qui n'osent pas parler librement devant eux. De même, elles se sentent suffisamment informées lors de leur 1ère consultation pour une contraception, alors que seule la moitié d'entre elles ont reçu des informations sur les MST, le préservatif et la pilule du lendemain. De même, moins de 20% sont informées des nouvelles méthodes contraceptives, ce qui explique leur faible diffusion à l'adolescence. Enfin, nous constatons que 5% de ces élèves ont déjà eu une IVG. La participation a été de 70% avec une bonne acceptation de la part des élèves et une liberté d'expression sur ce sujet qui n'est pas toujours retrouvé chez les adultes. Par ailleurs on note que la prévention des IVG aux Pays-Bas où le taux d'IVG est très bas, passe par un réseau d'information développé autour des médecins généralistes. Ceux-ci ont donc une place importante dans la prévention des IVG chez les adolescentes car ils les voient régulièrement et ils ont leur confiance. Néanmoins, il reste encore de nombreux points à améliorer : aborder de manière plus systématique le sujet de la sexualité t de la contraception, reçevoir les adolescents seuls en consultation plus régulièrement, donner plus d'informations lors de la prescription d'une contraception, s'adapter au mode de vie et aux besoins des adolescents en leur proposant tous les types de contraceptifs existants. Pour cela, il faudra également améliorer la formation des futurs médecins généralistes.

Book excerpt: A decade in preparation, Immigrant Entrepreneurs offers the most comprehensive case study ever completed of the causes and consequences of immigrant business ownership. Koreans are the most entrepreneurial of America's new immigrants. By the mid-1970s Americans had already become aware that Korean immigrants were opening, buying, and operating numerous business enterprises in major cities. When Koreans flourished in small business, Americans wanted to know how immigrants could find lucrative business opportunities where native-born Americans could not. Somewhat later, when Korean-black conflicts surfaced in a number of cities, Americans also began to fear the implications for intergroup relations of immigrant entrepreneurs who start in the middle rather than at the bottom of the social and economic hierarchy. Nowhere was immigrant enterprise more obvious or impressive than in Los Angeles, the world's largest Korean settlement outside of Korea and America's premier city of small business. Analyzing both the short-run and the long-run causes of Korean entrepreneurship, the authors explain why the Koreans could find, acquire, and operate small business firms more easily than could native-born residents. They also provide a context for distinguishing clashes of culture and clashes of interest which cause black-Korean tensions in cities, and for framing effective policies to minimize the tensions.
