Des phénomènes tels que l’allongement de la durée de la vie, l’urbanisation des populations, la difficulté d’entrée sur le marché du travail ou encore l’éclatement des modèles familiaux traditionnels marquent notre monde contemporain et impliquent la disparition d’anciennes solidarités et l’apparition de nouvelles solidarités redessinant les relations entre générations, posant alors le problème du sort des personnes les plus fragiles : les enfants, les malades, les handicapés et, surtout, les personnes âgées. – Quel est alors le rôle de la famille et des collectivités dans la protection de ces personnes ? – Quels rapports entre solidarités publiques et solidarités privées ? – Quels sont les droits et libertés reconnus aux personnes que l’âge, la maladie ou le handicap, placent en situation de dépendances ? Telles sont les questions au cœur de cet ouvrage. The importance of the human, economic and social issues caused by the question of generations’ solidarities led the International Society of Family Law to choose this theme for its XVIth World Congress (Lyon, July 19-23rd 2011). More than 200 speakers from 50 countries studied these questions from the legal angle, but also philosophic, economic and anthropological. This work collects a part of these papers about two great issues: the child, as the center of family solidarities; and the support for elders by family. Phenomena such as increasing life expectancy, population urbanization, labor-market entry barriers, decline of traditional family patterns, mark in depth our contemporary world and involve old solidarity disappearance and new solidarity emergence, reshaping relations between generations while bringing up the problem of the fate of the most vulnerable: children, the sick, disabled, and especially elderly people. – What then is the role of families and communities in protecting these people? – What is the relationship between public and private solidarity? – What are the rights and freedoms of people placed by age, illness or disability in a dependence situation? These are the issues addressed by the authors of this book.

Book excerpt: In the early stages of planning the Third International Conference in System Science in Health Care, the steering committee members, most of whom had participated in the first conference in Paris (1976) and the second in Montreal (1980), made some basic decisions about organization of subject matter. The earlier meetings had been very successful in bringing together specialists from the health professions and the traditional sciences. In addition to physicians and nurses, these were representatives of the disciplines of the behavioral sciences, system theory, economics, engineering, and the emergency fields of management science and informatics -all concerned with the development of health resources in a broad system context. The reported research and experience of the many disciplines represented had dealt with one or more of three concerns: 1) a major health problem, such as cardiovascular disease, or an important popUlation at risk, such as the elderly or children or workers; 2) some generic aspect of organization and decision making, including trial and evaluation ofinnovative health strategies; and 3) the methodology of research and analysis in system of health service. The challenge to the conference organizers lay in the eliciting and arranging of experiences in such a way that the health services could be seen as purposeful,living, evolving systems.

Mobilisant l'économique néo-institutionnelle, nous proposons une analyse de l'accessibilité des soins reconnaissant l'importance de l'accessibilité financière et intégrant au delà, les caractéristiques structurelles de l'offre de soins et les pratiques professionnelles. Nous soulignons alors les limites du dispositif CMU-C dans un système institutionnel et organisationnel inchangé. Un travail économétrique de régression logistique sur les bases de consommations médicales 2007 des bénéficiaires de la CMU-C (Rhône et Drôme) soutient ce positionnement. Une réflexion sur les apports et modalités de développement d'une offre de services de santé intégrée et pluri-professionnelle, à l’instar des centres de santé et des maisons de santé pluridisciplinaires, est conduite à partir d'un matériau riche issu d'entretiens semi-directifs auprès d'acteurs du secteur de la santé. Ces formes d'organisation s'avèrent être génératrices de coordination, continuité et globalité des soins. Néanmoins, leur développement dépend des choix internes aux organisations créant une réelle plus-value en matière de coordination et de coopération des professionnels ainsi que des règles et représentations institutionnelles portées par le système de santé. Une évolution conjointe des deux niveaux conditionne leur émergence en tant qu'organisations structurantes des soins primaires, leviers de réduction des inégalités de santé.
