Book excerpt: We study a patient-doctor-laboratory relationship in which the doctor's treatments are credence goods, clinical laboratory tests can provide information to mitigate issues arising from asymmetric information between the doctor and the patient, and the doctor plays the dual role of service provider and information intermediary. Kickbacks distort the doctor's incentive to prescribe tests. Patients who are unaware of kickbacks can be hurt by both over-provision and under-provision of lab tests. Banning kickbacks can harm social welfare since the positive effects of kickbacks outweigh their negative effects when under-provision of services specific to credence-goods markets is a severe problem. Educating patients helps reduce kickbacks, but a moderate level of social awareness instead of full awareness can be socially optimal.
