Stigma towards mental health and those who struggle with mental illness has played a key role in this crisis. The present study aimed to examine attitude change towards mental health using a psychoeducational intervention. Students at a Japanese university read one of three cultural letters (individualistic, collectivistic, and neutral), then rated their attitudes towards the mentally ill. Participants also rated how culturally congruent the letter they read was with Japanese culture. Results indicated a significant effect of the letters on authoritarian attitudes towards the mentally ill. Furthermore, the collectivistic letter was rated significantly more culturally congruent than the individualistic letter. An exploratory analysis of gender revealed that women were rated higher in benevolence attitudes both before and after reading the letter. These findings indicate that perhaps some domains of attitude towards mental health are malleable, and that dissemination of information about mental health could potentially lead to a reduction of stigma over time.

This is particularly true of the mentally ill. Stereotypes are most ingrained for mental pathologies with heightened visibility in society, such as schizophrenia. Preconceived notions about danger, occult powers and mysterious malevolence which hover over the illness, contribute to the total debasement of the patient. Persons suffering from other forms of mental illness are stigmatized to a lesser degree. But the threat is real that labeling will extend to every endeavor linked to mental illness: care facilities, professionals, therapies in general and psychotropic medication in particular. Lay belief in the existence of important side-effects to this medication and public fears about the risk of addiction form the basis of very restricted, or even hostile, attitudes towards it and result in weak compliance. Inversely, psychotherapy now seems widely accepted and different forms of intervention have contributed to de-stigmatizing psychiatric illness and to stop the exclusion of patients. This book is of interest not only to psychiatrists, but also to mental health workers, psychologists, social scientists and social workers who wish to alter common precepts and prejudices regarding psychiatric disorders.

Based on extensive original research including interview research with a range of practitioners involved in dealing with the phenomenon, the book outlines how hikikomori expresses itself, how it is treated and dealt with and how it has been perceived and regarded in Japan over time. The author, a clinical psychologist with extensive experience of practice, argues that the phenomenon although socially unacceptable is not homogenous, and can be viewed not as a mental disorder, but as an idiom of distress, a passive and effective way of resisting the many great pressures of Japanese schooling and of Japanese society more widely.
