Maximum voluntary hand-grip strength and time to reach maximum strength were measured on 34 Army personnel. All three instruction conditions yielded reliable measures of strength and time-to-maximum output. The variability of the data, however, differed appreciably among the instruction conditions. The results emphasize the necessity for explicit instructions to the subjects in strength assessment studies, and the importance of reporting in detail all factors which might influence the measure obtained.

Book excerpt: The purpose of this investigation was to determine the validity and reliability of the Nicholas Manual Muscle Tester (NMMT), a portable dynamometer, as a measure of the isometric strength in women with arthritis. Female subjects (N=13; 66 ± 13.89 yrs.) with arthritis were tested for isometric muscle strength on the shoulder and hip (abduction, adduction, flexion, extension). Subjects were tested on three separate days using NMMT and Kincom 500-H dynamometers. Each subject performed three maximal isometric contractions for each joint action. A visual analog pain scale was used to determine level of pain prior to testing. Reliability values based on intraclass correlations coefficients (R) ranged from .85 to .93., with the exception of shoulder abduction being .49. Validity was determined correlating the mean value of the NMMT score with corresponding Kincom isometric measure. Pearson product moment correlations ranged from (r) .02 to .86, with 4 of 8 values meeting .05 level of significance. Correlation coefficients for pain and isometric force values were inconclusive and ranged from -.305 to .218. Major conclusions were: a) NMMT had high test-retest reliability in this sample; b) NMMT provides little criterion evidence of validity with the Kincom for most movements of hip and shoulder; c) level of pain was not a significant factor in subject reliability.

Physical therapy, in particular, has benefited significantly from this technology, which rapidly became the tool of choice in hundreds of research papers as well as the cornerstone of quantitative muscle performance assessment in the clinical setting. About 1984, the technology behind isokinetic dynamometry made considerable progress when computers were incorporated to control the hardware, that is the integral power sources and the on-line processing of mechanical signals. This enabled users to establish a common basis for carrying out eccentric contractions and to obtain comprehensive information on muscle strength immediately. This book does not pretend to cover all the topics that have been examined under isokinetics. Nor is it intended to be a quick and superficial introduction to clinical applications. Rather, it is aimed at those who have at least some experience and are at a stage where they are beginning to ask some very serious questions and would not necessarily be happy with very simple answers.

Book excerpt: The reliability and validity of a novel isometric strength measurement device was found to be comparable to accepted commercial devices for elbow flexion and extension. Developed at UNB to test isometric elbow strength, the Limb Strength Measurement Device (LSMD), is a compact, accurate alternative to hand-held dynamometry (HHD) and isokinetic dynamometry (IKD). Simulated loading environments confirmed the reliability of the device’s measurement hardware and software in isolation, while indicating that the LSMD’s ability to self-stabilize was due in part to postural adjustments of the user. In situ analysis was based on a split-session, multiple-factors statistical design performed with 20 healthy adults. The resulting intersession, intra- and inter-rater reliability data showed that the LSMD was comparable to HHD and fixed-IKD. Modified Limits of Agreement plots and regression analysis provided validation of the LSMD compared to the fixed-IKD, yielding calibration formula that were capable of correcting for fixed and proportional measurement bias.
