Cette intervention qui a été tant décriée se déroule maintenant dans des conditions d'hygiène rigoureuses et dans un climat de confiance. L'IVG reste cependant une épreuve dans la vie d'une femme. Bien que les études menées en France montrent une augmentation des techniques et de l'utilisation des moyens contraceptifs, le taux d'IVG ne diminue pas. Une étude descriptive a été réalisée à partir de 180 dossiers de femmes âgées de plus de 35 ans afin d’évaluer si l'échec de contraception est la principale raison de ce taux d'IVG élevé. Il en ressort que de nombreuses femmes sont suivies régulièrement sur le plan gynécologique mais rencontrent des problèmes par rapport à l'usage de la contraception. En effet, nombreuses d'entre elles n'avaient pas de moyen contraceptif au moment du rapport fécondant. D'autres utilisent mal la contraception ou doivent faire face à un échec de contraception. Ainsi, on peut penser que la contraception n’est pas adaptée aux femmes lors de sa prescription. Il semble indispensable que les professionnels de santé soient sensibilisés à ce problème afin d'améliorer la prise en charge des femmes et surtout d'éviter les récidives. Cependant, les problèmes liés à la contraception ne semblent pas être les seules raisons du recours à l'IVG. Le désir d'enfant n'est pas toujours rationnel, il peut dépasser l'entendement et donc conduire à une demande d'IVG comme dernier recours.

Aujourd’hui, la possibilité de recourir à un avortement n’est pas offerte à toutes les femmes. Dans le Monde, une femme sur deux risque sa vie en choisissant de ne pas poursuivre une grossesse qu’elle n’a pas désirée. Des efforts de libéralisation sont en cours et des progrès restent à faire en termes de légal sation de l’Interruption Volontaire de Grossesse. En France, bien que légalisé depuis, 35 ans, le droit à l’avortement reste un acquis fragile pour les femmes. Les techniques sont de plus en plus fiables, sûre et faciles d’utilisation, mais le désengagement continu des établissements et des professionnels de santé en restreignent l’accès, et le parcours des femmes est plus long et plus compliqué lorsqu’elles s’adressent à leur médecin généraliste. La Seine-Saint-Denis a été le premier département de France ayant rendu possible la pratique de l’IVG par méthode médicamenteuse hors établissement de santé en 2004. Le médecin généraliste a désormais la possibilité de pratiquer l’IVG par méthode médicamenteuse utilisant l’association mifépristone/misoprostol, que ce soit en cabinet de médecine libérale ou en centre de planification. Une plus grande implication de sa part permettra de garantir la pérennité de l’avortement, d’améliorer la prise en charge des femmes et leur accès à l’IVG. Notre étude prospective menée en 2007, portant sur le vécu par les femmes de l’avortement médicamenteux, nous a permis de montrer que l’avortement médicamenteux peut ne pas être douloureux et que la durée de la douleur intervient plus que son intensité dans le vécu physique et psychologique de la méthode. Le protocole d’IVG médicamenteuse en médecine de ville doit pouvoir être amélioré, par l’administration systématique d’antalgiques à intervalles réguliers et par l’amélioration de l’intervalle de prise entre la mifépristone et le misoprostol par exemple.
