There are difficulties in distinguishing the criminal from the mad or, more euphemistically, the mentally ill. Controversy has centered on the frightening potential possessed by the state to deprive of his rights the individual officially classified as mad. In this book, Wing, a psychiatrist of international repute, argues for a limited medical definition of mental illness, although he explains how even a doctor's professional judgment may often be influenced by social pressures. He compares concepts of madness prevalent in different types of society, examining, for example, the Marxist attitude towards the deviant in a socialist state. In a chapter which draws much from his own experience, he shows precisely how the apparatus of state medicine is used to suppress political dissidence in Russia. He also critically reviews the petty tyrannies prevalent in the West and tackles the difficult analytical problem of schizophrenia, a subject on which he is one of the most respected medical authorities. Reasoning about Madness is an original and important work in which the author successfully resists the temptation to erect "grand theories that explain nothing because they attempt to explain everything." Instead, he concentrates on developing a definition of madness which strikes a balance between the benefits of medical care and the preservation of human liberties.

Through time and across cultures it has acquired many formulations: for some, madness is synonymous with unreason and violence, for others with creativity and subversion, elsewhere it is associated with spirits and spirituality. Among the different formulations, there is one in particular that has taken hold so deeply and systematically that it has become the default view in many communities around the world: the idea that madness is a disorder of the mind. Contemporary developments in mental health activism pose a radical challenge to psychiatric and societal understandings of madness. Mad Pride and mad-positive activism reject the language of mental 'illness' and 'disorder', reclaim the term 'mad', and reverse its negative connotations. Activists seek cultural change in the way madness is viewed, and demand recognition of madness as grounds for identity. But can madness constitute such grounds? Is it possible to reconcile delusions, passivity phenomena, and the discontinuity of self often seen in mental health conditions with the requirements for identity formation presupposed by the theory of recognition? How should society respond? Guided by these questions, this book is the first comprehensive philosophical examination of the claims and demands of Mad activism. Book excerpt: "This book is the result of years of research spent in archives and libraries on two continents in an attempt to decipher the textual footprints of asylum patients. Some of the results of this research have already been published in Carol Berkenkotter's book Patient Tales: Case Histories and the Uses of Narrative in Psychiatry, as well as in various journals. Here, we focus on tracing not just the patients' medical histories but also their life stories before they became patients and after they were discharged. We find that the diagnosis event is the watershed moment in their lives, and so we are looking for the textual--and textural--makeup of this decision. This was our own version of "starring the text," in the words of Alan Gross, of placing rhetorical analysis of the written word at the center of the web of cultural practices that made asylums possible in the nineteenth century; thus, we observe firsthand the psychiatric argumentation practices that led to diagnosis and the patients' efforts to counter those arguments. For a while we inhabited a world of fading calligraphy inscribed in esoterically paginated dusty tomes, amalgamated genres that also hosted occasional patient letters and artifacts (drawings, paintings, diagrams, objets d'art sometimes engraved in what appeared to be the patient's own blood). Whenever possible, case notes, certificates, and private correspondence were copied, transcribed, and analyzed (in some instance coded); and while we used various analytical frameworks, for the most part we let the texts guide us to what we hoped to be intelligible, plausible approximations of the embodied experience of mental illness for those who found themselves in an asylum" --

This is particularly true of the mentally ill. Stereotypes are most ingrained for mental pathologies with heightened visibility in society, such as schizophrenia. Preconceived notions about danger, occult powers and mysterious malevolence which hover over the illness, contribute to the total debasement of the patient. Persons suffering from other forms of mental illness are stigmatized to a lesser degree. But the threat is real that labeling will extend to every endeavor linked to mental illness: care facilities, professionals, therapies in general and psychotropic medication in particular. Lay belief in the existence of important side-effects to this medication and public fears about the risk of addiction form the basis of very restricted, or even hostile, attitudes towards it and result in weak compliance. Inversely, psychotherapy now seems widely accepted and different forms of intervention have contributed to de-stigmatizing psychiatric illness and to stop the exclusion of patients. This book is of interest not only to psychiatrists, but also to mental health workers, psychologists, social scientists and social workers who wish to alter common precepts and prejudices regarding psychiatric disorders.
