Book excerpt: A lo largo del trabajo se analizan las condiciones en las que se desenvuelve el sistema sanitario español desde una óptica de búsqueda de la eficiencia y dentro del entorno de los países desarrollados. Tras una introducción a la economía de la salud como rama del conocimiento, se estudian las condiciones de configuración de los distintos sistemas sanitarios en los países de la OCDE planteando siete esquemas teóricos que recogen las posibles estructuras existentes. Tras el análisis del entorno internacional nos centramos en el sistema sanitario español, analizando desde la óptica organizativa y de financiación sus principales características, sus similitudes y sus diferencias respecto a los sistemas de otros países europeos. dado que el tema central que planteamos es la necesidad de elevar el nivel de eficiencia de las organizaciones sanitarias, estudiamos una técnica de medición conocida como análisis envolvente de datos y llevamos a cabo una aplicación practica de la misma sobre los hospitales del servicio andaluz de salud durante el periodo 1993-1995 a partir de tres modelos distintos. dado que esta técnica exige el conocimiento de los inputs y outputs de las unidades evaluadas, dedicamos un capitulo al estudio de las dificultades que plantea la medida del output sanitario y a estudio de los distintos sistemas existentes en la actualidad. también relacionado con la aplicación practica, analizamos previamente las características del servicio regional de salud de la comunidad autonóma de Andalucía, puesto que constituye el marco organizativo y financiero en el que se encuentran situados los hospitales que analizamos. Los resultados obtenidos en la medición de los niveles de eficiencia en los hospitales del servicio andaluz de salud, muestran que en el periodo analizado se han producido una elevación del nivel medio, al mismo tiempo que una reducción en las diferencias entre hospitales. Esta variación positiva permite depositar nuestra confianza en la aplicación de las técnicas de gestión sanitaria recientemente implantadas tales como los contratos programa y el establecimiento de incentivos ligados al cumplimiento de objetivos.

Book excerpt: This report aims to gather the lessons learnt on the effects of HIT to costs and benefits that might be of use to organisations looking to develop and implement HIT programmes. This is a difficult exercise considering the multiple factors affecting implementation of an HIT programme. Factors include organisational characteristics, the kinds of changes being put in place and how they are managed, and the type of HIT system. The report finds that barriers to HIT implementation are still substantial but that some progress has been made on reporting the organisational factors crucial for the adoption of HIT. However, there is a challenge to adapt the studies and publications from HIT leaders (early implementers and people using HIT to best effect) to offer lessons beyond their local circumstances. The report also finds limited data on the cost-effectiveness of HIT.
