Book excerpt: This work introduces an Agent Based Model that Simulates Human Error (ASHE) to predict the probability of human error in a dynamic and holistic manner. There have been several points in history where the occurrence of human error has resulted in the loss of resources, time, and even human lives. It is a phenomenon that plagues multiple industries, is random, and can often be difficult to track. Due to its random nature, there have been several studies conducted within the field of Human Reliability Analysis that propose models that aim to understand and mitigate human error. These studies offer a variety of perspectives on the issue and outline multiple methods that can be used to avoid human error. A majority of these studies, however, focus on a specific field only. For example, The Technique for Human Error Rate Prediction (THERP) was developed to focus on human errors occurring within nuclear power plants and The Man-Machine Integration Design and Analysis System (MIDAS) was developed with a focus on the aviation industry. Additionally, there seems to be room for improvement when it comes to how dynamically these studies analyze human error. ASHE was developed to bridge these gaps by leveraging Agent-Based Modeling and Design Structure Matrices to simulate a worker attempting a task, each step at a time. The interconnection between each step within a task is used to determine the actions of the worker among other characteristics outlined by the Standardized Plant Analysis Risk-HRA (SPAR-H) method to predict human error probability for the task as a whole. This work discusses the working of the model and runs through a case study highlighting how results from the model can be used to generate insights about the design of the task.
