Book excerpt: Introduction : le trouble de l'acquisition de la coordination ou dyspraxie développementale fait référence à des déficits mineurs de la coordination motrice, d'origine développementale. Il peut se manifester chez l'enfant par des acquisitions retardées dans les habiletés motrices illustrées par une certaine maladresse et une vitesse d'exécution lente. Ce retard a des conséquences dans la réalisation des actes de la vie quotidienne. Sa prévalence est estimée à environ 6% des enfants d'âge scolaire. L'âge moyen du diagnostic est situé entre 7 et 10 ans. Pourtant un repérage plus précoce permettrait une meilleure adaptabilité sociale, scolaire puis professionnelle. L'objectif de ce travail est de rechercher les outils de repérage et d'aide au diagnostic du trouble de l'acquisition de la coordination qui pourraient être adaptés à l'exercice de la médecine générale. Méthodes : une revue systématique de la littérature a été menée dans les bases de données PubMed, Springer, Cairn info, Science Direct, Cochrane Library et Docdocpro entre décembre 2017 et mars 2018. La méthodologie de PRISMA-P a été suivie pour la réalisation de ce travail. Résultats : 18 études ont été incluses permettant d'évaluer une dizaine d'outils de repérage. Parmi eux, les questionnaires Little DCD-Q et ChAS-P/T ont présenté des propriétés psychométriques intéressantes notamment une bonne sensibilité, précision et fiabilité au cours du temps pour le Little DCD-Q ainsi qu'une bonne cohérence interne et validité pour la ChAS-P/T. Le test moteur standardisé M-ABC 2 a montré des qualités en termes de validité, fiabilité et reproductibilité. Le little DCD-Q est un questionnaire qui s'adresse aux parents d'enfants âgés de 3 à 4 ans et 11 mois alors que la ChAS-P/T, composée d'un questionnaire parental et d'un questionnaire à compléter par l'enseignant, est destinée à des enfants de 4 à 8 ans. Le M-ABC 2 permet l'évaluation, par un professionnel de santé, des habiletés motrices d'enfants âgés de 3 à 16 ans et 11 mois selon trois tranches d'âge. Conclusion : parmi les outils disponibles, il existe des questionnaires qui interrogent les parents de l'enfant, dont l'application en médecine générale semble être prometteuse. Leur faisabilité dans un contexte de soins primaires nécessiterait toutefois d'être étudiée.

Chez certains enfants, cependant, les habiletés motrices ne se développent pas de façon appropriée. Pour nombre de ces enfants, aucune étiologie d'ordre médical ou neurologique ne peut être trouvée. Ils sont diagnostiqués comme atteints d'un Trouble de l'Acquisition de la Coordination (TAC). Les enfants atteints de TAC sont l'objet de ce livre. Ce trouble est désigné sous diverses appellations telles que la maladresse, la dyspraxie ou difficultés d'intégration sensori-motrice. Cet ouvrage a pour objectif d'informer le lecteur - qu'il soit soignant, chercheur, enseignant ou parent - des derniers acquis de la recherche portant sur le TAC. Il passe en revue les difficultés rencontrées par l'enfant souffrant d'un TAC (Geuze), les principaux tests disponibles dans le domaine des fonctions perceptivo-motrices (Albaret & Castelnau), les différents sous-types et la comorbidité (Visser), les déficits sous-jacents au TAC (Wilson & Butson), les principales approches thérapeutiques et leur efficacité (Polatajko & Cantin), et enfin, la prise en charge du TAC (Sugden). Ce livre souhaite favoriser la diffusion des connaissances scientifiques actuelles ainsi que l'utilisation des outils appropriés pour le diagnostic, l'évaluation et le traitement du TAC. L'objectif ultime est le développement optimal - non limité au seul développement moteur - de l'enfant atteint d'un TAC, ce qui constitue une gageure pour les parents, les enseignants et les professionnels concernés. L'originalité du sujet nous conduit à publier cet ouvrage simultanément en français et en anglais.

Malgré leur impact sur l'autonomie de l'enfant, les troubles d'utilisation impliqués dans ces activités quotidiennes demeurent peu explorés (e.g., mécanismes sous jacents, profils inter-individuels, recul évolutif). A notre connaissance, la notion d'utilisation d'outil n'a pas fait l'objet d'une modélisation développementale spécifique.Elle se situe aujourd'hui au carrefour des modèles sur le développement perceptivo-moteur et des théories sur l'émergence des capacités cognitives de résolution de problème chez l'enfant. Des modèles récents développés chez l'adulte apraxique suggèrent que l'utilisation d'outil est le fruit d'un processus dialectique entre un raisonnement technique (i.e., analyse abstraite des moyens techniques pertinents pour un but donné) et des habiletés sensori-motrices (i.e., guidées par les représentations sur les transformations mécaniques à opérer). A l'appui de ces modèles, la présente thèse propose d'une part d'analyser le développement typique de l'utilisation d'outil et des processus sous-jacents.Nous explorons d'autre part l'hypothèse d'un développement atypique de l'utilisation d'outil chez des enfants diagnostiqués TAC. Nos résultats apportent des arguments en faveur de la contribution du raisonnement technique au développement typique de l'utilisation d'outil. Sur le plan clinique, l'analyse de l'étendue et des causes sous-jacentes des difficultés d'utilisation d'outil confirme l'importance d'un tel examen pour la validation des critères diagnostiques du TAC.
