Book excerpt: Parent perception of the cost and value of grommets in the management of otitis media with effusionBackgroundOtitis Media with effusion (OME) in children is generally a self-limiting condition and consequently, its treatment, particularly grommet insertion, is considered of limited clinical effectiveness by commissioners.Numerous studies have shown high patient satisfaction following treatment with grommets. However, no study has used contingent valuation such as the financial value patients or carers assign to this treatment.Objectivesu00a5tTo determine parent/carer awareness of the NHS cost of grommet insertion and related outpatient appointments.u00a5tTo establish the financial value that parents assign to their childu2019s treatment of OME.Methodsu00a5tA 2-part questionnaire was given to parents of children with OME prior to and following surgery; this was a pilot of 10 patients. Questionnaires recorded demographic information and careru2019s cost perception of treatment.u00a5tA retrospective survey was sent to 103 patients exploring careru2019s cost perception of treatment. Parents who did not return the postal survey were contacted by telephone.ResultsThe response rate for the retrospective survey was 12% (13). Due to the low numbers, the data sets from both parts of the study were analysed together. Therefore, the total number of respondents was 23.Parents were given information on the NHS tariffs for outpatient appointments and grommet insertion. 100% considered that grommets are worth the expense for the NHS. 47% of parents thought that the tariff for grommets was less than they expected (37% same, 16% more). 21% of parents thought that the tariff for outpatient clinic was less than expected (47% same, 32% more).ConclusionParents of children undergoing grommets for OME have little awareness of the true cost of clinic appointments and surgical procedure. Although they grossly overestimated their value, they were unwilling to match the NHS tariffs.Unfortunately, the low response rate meant that meaningful correlation between valuations and carer demographics was not possible.With current pressures on a universal health service free at the point of delivery, we believe that rationing services is inevitable. Based on data compiled from a large number of prospective and longitudinal studies, this volume evaluates the course, and some of the causes, of the condition and demonstrates morphological differences in the nasopharynx of affected children. Included in this analysis is a report on the effects of surgical management with adenoidectomy, adeno-tonsillectomy and grommets. Complications that arise from the disease and its treatment are reported along with factors of prognostic significance for outcome. Evaluation of these factors has enabled the author to make suggestions of factors to help define where surgery would be beneficial.
