Book excerpt: Las neumonías relacionadas con la ventilación mecánica (NVM) son una de las infecciones más frecuentes y graves en los pacientes críticos ingresados en los Servicios o Unidades de Cuidados Intensivos (UCI). A lo largo de los años se han realizado numerosos estudios para identificar los factores de riesgo asociado a esta infección. Sin embargo, su relación con las características estructurales y funcionales de los hospitales ha sido un aspecto que no ha sido estudiado. En este trabajo (motivo de la tesis doctoral) se ha analizado la relación de las tasas de infección de las NVM (expresadas como densidad de incidencia), su etiología y la presencia de bacterias multirresistentes (BMR) con respecto a la estructura y función hospitalaria definida por tamaño hospitalario (200, 200-500 y 500 camas), gestión y/o administración (hospitales públicos o privados) y docencia pre y/o postgrado (hospitales docentes y no docentes) con la hipótesis de que las características estructurales y funcionales de los hospitales influyen en la frecuencia, etiología y presencia de BMR de las NVM. Para demostrar la hipótesis se han utilizado los datos de los pacientes incluidos en el registro "Estudio Nacional de Vigilancia de Infecciones Nosocomiales" (ENVIN) en UCI de la Sociedad Española de Medicina Intensiva, Crítica y Unidades Coronarias (SEMICYUC), entre los años 2007-2013 (117.354 pacientes) en la que han participado más de 200 UCI española. Las NVM se han diferenciado por su etiología y presencia de bacterias multirresistentes (BMR) identificadas como Pseudomonas spp. multirresistente (resistente a tres o más familias de antibióticos), Acinetobacter spp. resistente a imipenem, enterobacterias resistentes a cefotaxima/ceftriaxona, enterobacterias resistentes a imipenem, Stenotrophomonas spp. resistente a cotrimoxazol y Staphylococcus aureus resistente a meticilina (SARM). Así mismo se han analizado 21 variables relacionadas con las características de los pacientes que sirvieron para describir el case-mix para cada uno de los subgrupos de pacientes analizados. Finalmente se realizó un análisis multivariado para identificar las variables relacionadas de forma independiente con la presencia de BMR en las NVM. De esta forma se han seleccionado 4.914 NVM que se utilizaron para identificar las tasas de incidencia (episodios de NVM por 1.000 días de ventilación mecánica) y microbiología asociada. En 3.911 casos se disponía de un diagnóstico bacteriano de seguridad y de ellos, en 3.555 NVM se disponía del antibiograma respecto del germen responsable, siendo este grupo el que se utilizó en los estudios de resistencia.
