Book excerpt: This quantitative study explored perceptions, knowledge and beliefs of parents regarding enrolling their adolescent children in School-Based Health Centers (SBHCs) and the influence that adolescents have on this decision. The Health Belief Model was the theoretical basis. Literature measuring parents' knowledge and perceptions of adolescent SBHC services and the influence teens have on health care decisions made on their behalf is negligible. Parents of 9-12 grade students attending two regional schools in Connecticut with SBHCs were recruited and surveyed through school events and by email. The sample (n = 57) was White (>42%), Hispanic (>31%), Black (>24 %) and Mixed Race (1.8%). Over 38% had incomes of at least 130% of the poverty level. Most spoke English and 53% were enrolled in the SBHC. Students were more likely to be enrolled in the SBHC if they received free/reduced lunch, or when parents were familiar with health care recommendations for teens, understood the benefits of SBHCs or knew someone also enrolled. When teens asked to enroll in the SBHC parents were more likely to consent. Most parents were aware of risk factors affecting teens, but this did not translate into higher enrollment. Parents may not be aware that SBHCs screen and provide services to address these risk factors. Parents may also believe that SBHCs are only for those of lower SES. SBHCs must promote the benefits of enrollment, emphasize that all students are eligible for services, educate parents and students on their full range of services, and do so in ways that reach parents not traditionally reached. Parents and students who have had a positive experience can be an asset in recruitment efforts.

GAO found that: (1) SBHC afford children easier access to needed health services by bringing providers to the children, furnishing free or low-cost services, and supplying the atmosphere of trust and confidentiality adolescents need; (2) SBHC do not provide all health services required by students and cannot reach adolescents who have dropped out of school; (3) a lack of stable financing is a major concern for SBHC, with some centers reporting insufficient funds to meet all children's service needs; (4) SBHC often have difficulty obtaining reimbursement from public and private insurers, including Medicaid; (5) SBHC have difficulty recruiting and retaining appropriately trained nurse practitioners and physician assistants; (6) community debates over the appropriateness of providing reproductive health services in SBHC have limited the centers' ability to meet some adolescents' health needs; (7) communities lack access to information on establishing new centers and solving problems at existing SBHC; (8) research measuring the impact of SBHC on students' health and education is sparse; and (9) although efforts to coordinate school health programs within the federal government have begun, the Department of Health and Human Services does not have a focal point to answer outside inquiries, provide technical assistance, or develop a research agenda.

From birth, children are learning and rely on parents and the other caregivers in their lives to protect and care for them. The impact of parents may never be greater than during the earliest years of life, when a child's brain is rapidly developing and when nearly all of her or his experiences are created and shaped by parents and the family environment. Parents help children build and refine their knowledge and skills, charting a trajectory for their health and well-being during childhood and beyond. The experience of parenting also impacts parents themselves. For instance, parenting can enrich and give focus to parents' lives; generate stress or calm; and create any number of emotions, including feelings of happiness, sadness, fulfillment, and anger. Parenting of young children today takes place in the context of significant ongoing developments.
