It authorizes commander deployment status decisions for specific Medical Readiness Classification and deployment-limiting codes; incorporates Army Directive 2018-11, Update to Redesign of Personnel Readiness and Medical Deployability; incorporates Army Directive 2019-07, Army Dental Readiness and Deployability; updates individual medical readiness classification; describes that temporary profiles no longer have assigned physical capacity or stamina, upper extremities, lower extremities, hearing and ears, eyes, psychiatric designation; and redesigns and prescribes the DA Form 3349 (Physical Profile Record) as a single source incorporating all duty limiting conditions and current functional limitations for providers, commanders, and trained staff. Unit commanders will review profiles on Soldiers under their command and make a determination for deployability for all duty limiting conditions not identified by policy. This regulation requires a physician review and second signature for all permanent profile with a serial of "2. It implements DODI 6025.19 and DODI 6490.07; the Assistant Secretary of Defense for Health Affairs memorandum, Subject: Individual Medical Readiness Measure Goal, dated July 15, 2015; and supplements the information provided in AR 220-1 (throughout). It implements the Commander Portal and clarifies required actions to support Soldier health and welfare, duty assignment, and medical readiness reporting (throughout). It incorporates Army Directive 2016-07, Redesign of Personnel Readiness and Medical Deployability.

Book excerpt: The health of the force is among the most important indicators of Army readiness and is critical to both the present and future readiness of the force. The Army defines individual medical readiness (IMR) as the ability to achieve medical fitness standards within 72 hours of deployment. Future health readiness focuses on Soldier wellness, maximization of holistic health, and prevention of chronic disease and injury. Improvements in future health readiness should increase rates of IMR, improve the health of the force, and decrease DoD and Army healthcare costs. Army doctrine, systems, and programs such as the System for Health, the Performance Triad, and the Army Wellness Centers provide a strong framework for Army health promotion. Improvements in Army doctrine, systems, programs, and policies as well as further alignment with wellness industry best practices will increase future health readiness in the Army and provide cost savings to the Military Health System.

Book excerpt: The purpose of this study is to determine the state of physical and medical readiness of US Army Reserve troops mobilized for Operations by identifying those medical conditions leading to either requests for medical waivers or those designated medical non-available for deployment status. The Army Reserve plays a crucial complementary role to the active component in times of national crises, in particular, in the support areas of medicine. With this responsibility, however, comes the requirement that when the Army Reserve Soldier is mobilized she/he is fit for duty. To assure that Army Reserve Soldiers are in a state of medical and physical readiness when mobilized, their reserve unit is individually tasked with guaranteeing soldier compliance in specific areas of health, weight and physical fitness. These units through scheduled soldier readiness processing (SRP) checklists also must maintain individual medical records with updated information. These data are used to determine the overall fitness of the individual soldier for deployment. A myriad of medical conditions that are defined in Army Regulation (AR) 40-501 could disqualify soldiers with permanent 3 or 4 profiles (P3, P4) for worldwide deployment and require immediate follow-up action by their reserve unit.

This report describes (1) the extent that DOD's policies for assessing individual medical readiness include unique health care issues of female servicemembers; (2) the availability of health care services to meet the unique needs of female servicemembers at domestic Army installations; and (3) the extent that DOD's research organizations have identified a need for research on the specific health care needs of female servicemembers who have served in combat.GAO reviewed DOD and military-service policies on individual medical readiness and surveyed senior health care officials about the availability of specific health services at the 27 domestic Army installations with MTFs that report directly to the domestic regional medical commands. Book excerpt: As the United States Army shifts from a forward presence force to power projection force, the most challenging mission facing the Army Medical Department will be to maintain a medical force able to deploy in support of multiple contingency operations and continue to provide health care to Continental United States (CONUS) eligible beneficiaries. To answer this challenge, the Army Medical Department has developed the Caretaker Hospital concept. Under this program, active component hospitals remain in the force structure in a cadre status. The hospitals are at full authorized levels of organization (ALO) for equipment and very low ALO for personnel. They are reduced in full time manpower authorizations to a limited cadre with the residual health care staff assigned to a peacetime health care facility. The equipment is configured for long term, depot storage in order to ease deployment. This study examines the proposed operational concept and training plan for the Caretaker Hospital program.

Book excerpt: The U.S. Department of Defense (DoD) faces short-term and long-term challenges in selecting and recruiting an enlisted force to meet personnel requirements associated with diverse and changing missions. The DoD has established standards for aptitudes/abilities, medical conditions, and physical fitness to be used in selecting recruits who are most likely to succeed in their jobs and complete the first term of service (generally 36 months). In 1999, the Committee on the Youth Population and Military Recruitment was established by the National Research Council (NRC) in response to a request from the DoD. One focus of the committee's work was to examine trends in the youth population relative to the needs of the military and the standards used to screen applicants to meet these needs. When the committee began its work in 1999, the Army, the Navy, and the Air Force had recently experienced recruiting shortfalls. By the early 2000s, all the Services were meeting their goals; however, in the first half of calendar year 2005, both the Army and the Marine Corps experienced recruiting difficulties and, in some months, shortfalls. When recruiting goals are not being met, scientific guidance is needed to inform policy decisions regarding the advisability of lowering standards and the impact of any change on training time and cost, job performance, attrition, and the health of the force. Assessing Fitness for Military Enlistment examines the current physical, medical, and mental health standards for military enlistment in light of (1) trends in the physical condition of the youth population; (2) medical advances for treating certain conditions, as well as knowledge of the typical course of chronic conditions as young people reach adulthood; (3) the role of basic training in physical conditioning; (4) the physical demands and working conditions of various jobs in today's military services; and (5) the measures that are used by the Services to characterize an individual's physical condition. The focus is on the enlistment of 18- to 24-year-olds and their first term of service.

Book excerpt: The more than 200,000 men and women that make up the Department of Homeland Security (DHS) workforce have been entrusted with the ultimate responsibility - ensuring that the homeland is safe, secure, and resilient against terrorism and other hazards. Every day, these dedicated individuals take on the critical and often dangerous challenges of the DHS mission: countering terrorism and enhancing national security, securing and managing the nation's borders, enforcing and administering U.S. immigration laws, protecting cyber networks and critical infrastructure, and ensuring resilience in the face of disasters. In return, DHS is responsible for protecting the health, safety, and resilience of those on whom it relies to achieve this mission, as well as ensuring effective management of the medical needs of persons who, in the course of mission execution, come into DHS care or custody. Since its creation in 2002, DHS has been aggressively addressing the management challenges of integrating seven core operating component agencies and 18 supporting offices and directorates. One of those challenges is creating and sustaining a coordinated health protection infrastructure. Advancing Workforce Health at the Department of Homeland Security examines how to strengthen mission readiness while better meeting the health needs of the DHS workforce. This report reviews and assesses the agency's current occupational health and operational medicine infrastructure and, based on models and best practices from within and outside DHS, provides recommendations for achieving an integrated, DHS-wide health protection infrastructure with the necessary centralized oversight authority. Protecting the homeland is physically and mentally demanding and entails many inherent risks, necessitating a DHS workforce that is mission ready. Among other things, mission readiness depends on (1) a workforce that is medically ready (free of health-related conditions that impede the ability to participate fully in operations and achieve mission goals), and (2) the capability, through an operational medicine program, to provide medical support for the workforce and others who come under the protection or control of DHS during routine, planned, and contingency operations. The recommendations of this report will assist DHS in meeting these two requirements through implementation an overarching workforce health protection strategy encompassing occupational health and operational medicine functions that serve to promote, protect, and restore the physical and mental well-being of the workforce.
