Book Human Centred Design in Laparoscopic Skills Acquisition Shifting Paradigms in the Age of Technology

Download or read book Human Centred Design in Laparoscopic Skills Acquisition Shifting Paradigms in the Age of Technology written by Sandeep Ganni and published by . This book was released on 2020-08-27 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Minimal Access Surgery (MAS) has multi-faceted implications on the different stakeholders involved in implementation. It requires the surgeon to cope with the ergonomic and cognitive challenges required to perform a surgical procedure. It needs the surgical team to work coherently for the smooth functioning of the technologically complex operating room (OR). It needs educators and policy makers to embrace reform into the novel practices of training skills in MAS. It needs the industry to engage in research to bring out newer tools and technologies akin to training tools. This thesis explores the premise of implementation of MAS in the areas of curriculum design, implementation of training protocols, assessment tools and emerging technologies and trends that improve the learning curve and the transfer of skills from a skills lab setting to the OR. Several surgeons were included from different parts of Europe and India with expertise levels ranging from novice to expert. To study the unique requirements of individual surgeons and the detriments of performance the Laparoscopic Surgical Skills (LSS) Grade 1 Level 1 was used as a common training curriculum. First, a worldwide survey was conducted on the variety of training modalities and tools. Then, Self-assessment and the implications of "reflection-before-practice" on skills progression was studied. Automated assessment tools were devised using motion tracking software and thresholds for optimal performance were proposed. Physiological markers were studied during OR performance to determine factors that influence immersion in virtual reality surroundings and replicated in physical space using 3D projection for team training. Evolving technology in MAS requires multifaceted approach from training to customization to ensure optimal and efficient patient and ergonomic outcomes.

Book Methods for laparoscopic instrument tracking and motion analysis for objective assessment of surgical technical skills

Download or read book Methods for laparoscopic instrument tracking and motion analysis for objective assessment of surgical technical skills written by Juan A. Sánchez-Margallo and published by GRIN Verlag. This book was released on 2015-03-19 with total page 214 pages. Available in PDF, EPUB and Kindle. Book excerpt: Doctoral Thesis / Dissertation from the year 2014 in the subject Medicine - Biomedical Engineering, grade: Excellent "cum laude", , language: English, abstract: Minimally invasive surgery (MIS) has become in many surgical specialties and procedures the gold-standard choice due to its efficiency and benefits towards patient safety. However, the introduction of laparoscopic surgery has led to the need of developing new surgical skills different to those required for open surgery, with a significant learning curve to perform a safe laparoscopic surgery. Traditional subjective assessment methods of trainees are no longer adequate for surgical training. Reduced working hours as well as demands from surgeons and associations mean that more objective assessment tools that can accredit surgeons as technically competent are required. Evidence exists to validate motion analysis for laparoscopic technical skills assessment. Motion analysis of the laparoscopic instruments seeks to determine aspects that indicate the difference between surgeon's level of surgical dexterity. However, at the moment there is not an extended method to be used with current available training systems used in training laboratories as well as for the OR. The purpose of this book is to present the design, development and validation of three novel motion analysis methods focused on the use of real laparoscopic instruments during laparoscopic performance. These methods are based on computer vision techniques attempting to not interfere with the surgical practice. They are introduced in an evolutionary way from methods for exclusive use in a box trainer to solutions with the potential of being used in actual OR setting. Overall, this work corroborates the research hypothesis regarding the use of three different video-based tracking technologies for motion analysis of laparoscopic instruments, the use of instrument motion analysis for MIS technical skills assessment and the relationship between motion-related assessment metrics and quality of technical performance in laparoscopic training. These presented methods provide a tool to objectively assess MIS technical performance and a support to train novice surgeons in MIS techniques. The findings of this work encourage us to continue researching in improving these methods to be introduced as part of an actual laparoscopic training program, both inside and outside the OR.

Book The Laparoscopic Surgery Revolution

Download or read book The Laparoscopic Surgery Revolution written by David W. Page MD and published by Bloomsbury Publishing USA. This book was released on 2017-02-13 with total page 313 pages. Available in PDF, EPUB and Kindle. Book excerpt: Written by a surgeon with 40 years' experience in practice and instruction, this book provides vital, up-to-date information that explains the strengths and weaknesses of the laparoscopic surgery field to enable potential surgical patients to make the best decisions and choose a surgeon wisely. More than 30 years ago, laparoscopic or "keyhole" surgery suddenly appeared as an operative technique. Laparoscopy quickly grew in the U.S. surgical field, where now more than two million operations annually use the technique. But is the training surgeons receive in laparoscopy sufficient to ensure patient safety? What are the specific situations where laparoscopy is beneficial and justified, and when is it ill-advised due to the additional complexity and risk factors? This is the first book written for general readers—avoiding medical jargon wherever possible—to expose the gritty history and downsides of "minimally invasive surgery." Additionally, it provides the perspective and insights of an esteemed surgeon who was working at the inception of laparoscopy and has a full understanding of this now widely popular procedure across its development and lifespan. Readers will learn about the emergence of laparoscopic techniques in the 1990s, understand how minimally invasive surgery has been a boon to the business of surgery and to patient health and recovery overall, appreciate how the complexity involved in laparoscopic surgery has led to a higher incidence of surgeon "incompetence," and grasp the responsibility of a patient to take steps to assure that the surgeon is qualified before going into the operating room. Examination of eye-opening statistics on the outcomes of laparoscopic procedures documents the high level of capability of most surgeons, as well as the lack of appropriate ability with certain laparoscopic operations in the hands of some surgeons. The author also spells out how informed patients can be prepared to discuss and consider all aspects of an operation—and the surgeon's training and experience—to assure the best outcome for their health.
