Le recours à la chirurgie muco-gingivale pédiatrique s'avère pourtant souvent nécessaire dans certaines situations cliniques que nous détaillerons dans ce travail. Les spécificités de ces interventions sont liées aux phénomènes de croissance et de maturation tissulaire parodontale, ainsi qu'à certaines pathologies et anomalies propres à l'enfant. Le dépistage et le traitement chirurgical précoce des anomalies dento-gingivales chez l'enfant et l'adolescent permettraient de s'affranchir d'interventions tardives chez l'adulte. Dans cette thèse nous aborderons essentiellement les indications pré, per et post-orthodontique de la chirurgie parodontale chez l'enfant. Aujourd'hui, les techniques chirurgicales de dégagement et de traction orthodontique de dents incluses trouvent de nombreuses indications, mais nécessitent souvent des chirurgies d'aménagement secondaire à type de greffe de tissu conjonctif ou de lambeaux déplacés, visant à obtenir une éruption sur l'arcade dans un contexte parodontal sain. Les frénectomies et les fibrotomies supra-crestales, dont nous détaillerons les indications et protocoles chirurgicaux, constituent d'autres chirurgies pré-orthodontiques qui seront abordées dans ce travail. Ainsi la chirurgie muco-gingivale chez l'enfant et l'adolescent peut s'avérer à la fois thérapeutique, interceptive et préventive et ses indications et protocoles doivent tenir compte de particularités anatomique, physiologique et psychologique. Nous aborderons donc dans cette thèse les particularités anatomiques du parodonte en denture temporaire et mixte en comparaison du parodonte chez l'adulte, mais également ses aspects développementaux et histologiques, afin de distinguer une évolution physiologique d'une évolution pathologique. Nous développerons également les indications générales et spécifiques des différents traitements chirurgicaux parodontaux, ainsi que leurs spécificité en terme de protocoles chirurgicaux chez l'enfant et l'adolescent. Dans une dernière partie, nous illustrerons ces propos par la description des phases pré-per et post-opératoires de différents cas cliniques pris en charge au sein d'une équipe multi-disciplinaire du Pôle dentaire des HUS.
