The purpose of this project was to educate providers and increase knowledge in barriers to proper antibiotic prescribing. The survey instrument used to assess provider knowledge on antibiotics was Antibiotic knowledge Attitude and Practices Survey (KAP). A retail clinic in New Jersey identified the problem of barriers to proper antibiotics prescribing. The target population was clinic providers. Clinic providers included all female nurse practitioners. Based on the problem, an extensive literature search showed that multifaceted interventions increased provider awareness to barriers to proper antibiotic prescribing. A pretest survey was implemented prior to the educational intervention to access prior knowledge. After the educational intervention, a posttest survey was completed to compare changes in knowledge. After seven weeks, a second posttest was performed to assess retention of knowledge. The project demonstrated that after educational intervention, participant's knowledge on barriers to proper antibiotic prescribing increased by 33.3%. The evidence-based practice change project demonstrated that education on barriers to proper antibiotic prescribing have to be offered to all providers to decrease the misuse of antibiotics. Book excerpt: Background: Cervical cancer is one of the most common cancers in the United States. A low education level and socioeconomic level are the commonality among women with cervical cancer. The disease affects Latina and African American women the most. Cervical cancer screening is one of the key preventive methods in decreasing cervical cancer rates as a cure rate is high with early detection. The project was conducted in an underserved community with a high population of African Americans and Latinos in Los Angeles, California. EBP Framework: An evidence-based practice model used for the project was "Model for Change to Evidence-Based Practice" by Rosswurm and Larrabee. The model guided the project to improve cervical cancer screening rates. Methods: The project objective included improving the number of cervical cancer screenings among wimen ages between 35 to 44-year-old. Rocky Mountain University of Health Professions approved the project through the Institutional Review Board and determined the project was not human subject research. A convenience sample of nurse practitioners (n=7) was selected and educated on cervical cancer screening and poster reminders within the clinic. The provider's confidence level and knowledge level were assessed using pre- and post- assessments for each. The outcomes were assessed using a chart review of four weeks during pre- and post-intervention. Findings/Results: The number of cervical cancer screenings, the providers' confidence levels, and knowledge levels were assessed during pre- and post-intervention phases. The outcomes suggested that the provider education and poster reminder with the use of the EBP model increased cervical cancer screening rates, the providers' confidence and knowledge levels. Conclusion/Recommendations: The project's outcomes suggested an increase in cervical cancer rates, the providers' knowledge and confidence level on cervical cancer, and the financial benefit of the increased cervical cancer screening rates. The clinic decided to continue using the poster reminders and possibly modify the poster to increase patients' awareness. The clinic is considered periodical provider education sessions.

Book excerpt: While Papanicolaou (Pap) screening has been implemented since the 1950’s, and is linked to decreased rates of cervical cancer, national screening rates are still below the national target screening rate. In Contra Costa County (CCC), the current cervical cancer screening rate is 50.92% (Contra Costa Health Services [CCHS], 2016a). This is far below the national goal of 93% and the first benchmark of 54.33% needed in order to receive state funding under the California Department of Health Care Services (DHCS) Medi-Cal 2020 waiver. The Medi-Cal 2020 waiver has a 5-year Public Health Incentives and Redesign in Medi-Cal (PRIME) program requiring participating health systems to complete projects that improve population health, such as increasing cancer screening rates. CCHS needs to increase screening rates in order to receive funding from DHCS. In this paper, the author of this Doctor of Nursing Practice (DNP) project discusses the design and implementation of an evidence-based cervical cancer screening toolkit to increase cervical cancer screening rates. The toolkit includes PowerPoint about the PRIME program and cervical cancer screening guidelines for Public Health Clinic Services (PHCS) staff, a pocket reference for cervical cancer screening, and a patient handout. During the two months following implementation of the toolkit, cervical cancer screenings for the PHCS pilot group increased when compared to the same time the prior year. The toolkit helped educate providers and patients, meeting the goal of increasing staff and patient knowledge about cervical cancer screening. Overall, the toolkit is a cost effective way to assist the efforts to increase cervical cancer screening rates and obtain Medi-Cal funding.

Book excerpt: This dissertation considers an evaluation of the health education and patient navigation (PN) intervention, Friend to Friend plus Patient Navigation Program (FTF+PN). In 2010, the Texas A & M AgriLife Extension Service was awarded outreach education funding by the Cancer Prevention Research Institute of Texas (CPRIT) to adapt the evidence-based program, Friend to Friend (FTF) in rural and border counties in Texas. FTF consists of “pink parties” intended for an audience of lower income, un-/underinsured women aged 40+ who may be disabled, self-employed, and/or have limited English proficiency (LEP). Increased funding in 2012 supported the addition of four, fulltime equivalent patient navigators to join the team of four, full-time equivalent regional cancer prevention specialists to allow for follow-up and active support for women to obtain the screenings. FTF+PN seeks to build an effective, sustainable infrastructure and overcome barriers to breast and cervical screening and diagnostic services to increase screening rates for underserved, un-/under-insured, and older women in approximately 60 rural and border counties. The goal is to increase the number of women screened according to American Cancer Society (ACS) guidelines for breast and cervical cancer, thereby increasing the probability any cancers detected would be diagnosed in earlier stages. At the time, ACS guidelines recommended annual mammograms for women aged 40-54 and biannual mammograms for those aged 55+ with average risk of breast cancer. For cervical cancer screenings, recommendations included Pap tests every 3 years for women aged 21-29 and every 5 years for women aged 30-65 with no additional screenings needed for women aged 65+ if their previous results were normal. The goal of this evaluation is to demonstrate the efficacy of combining PN, a patient-centered healthcare delivery model that utilizes trained lay navigators to integrate a fragmented system of care in order to reduce barriers to timely care for individuals and subsequently reduce disparities for population groups, with a health education intervention adapted for rural and border Texas. Screening outcomes are also evaluated in light of county-level poverty rates and educational attainment to provide more comprehensive statistical models advancing scientific understanding of screening behavior among varying groups of women.

This paper offers women and Advanced Practice Nurses an educational program grounded in review of evidence-based research and guidelines that will assist in providing female patients with screening methods to detect breast cancer and modifiable risk factors that will assist in decreasing the likelihood of disease. A form of malignancy that affects the cells of the breast, breast cancer is diagnosed in one of eight women during their lifetime. Breast cancer is a serious condition that reaches the lives of all members of the female population as up to 85% of women with newly diagnosed breast cancer do not have a family history of the condition. Women of all ages and ethnicities are being diagnosed with breast cancer with many of them unaware and uneducated concerning the prevention methods and lifestyle modifications that assist in decreasing the chance of developing the condition. Nurse practitioners play a crucial role in breast cancer prevention in women. As Advanced Practice Nurses continue to become an increased and strong presence in primary care provision for the population, they are able to provide teaching and education to women regarding their health and wellness and the prevention of breast cancer. The purpose of this paper is to provide an educational program for breast cancer prevention in women grounded in evidence-based research and guidelines concerning women’s health. This educational program will assist practitioners in educating the target population of women in identifying, understanding and practicing the necessary interventions for breast cancer prevention. By utilizing this educational tool, Advanced Practices Nurses will be able to ensure successful breast cancer prevention in their female patients. Effective breast cancer prevention will be achieved by educating and encouraging women to complete important screenings such as BRCA testing and mammography, addressing modifiable risk factors such as obesity, alcohol and tobacco use and implementing interventions such as exercise and healthy eating.

The main aim of the Guide is to boost implementation research capacity as well as demand for implementation research that is aligned with need, and that is of particular relevance to health systems in low- and middle-income countries (LMICs). Research on implementation requires the engagement of diverse stakeholders and multiple disciplines in order to address the complex implementation challenges they face. The second half of the Guide (5-7) detail the various methods and study designs that can be used to carry out implementation research, and, using examples, illustrates the application of quantitative, qualitative, and mixed-method designs to answer complex questions related to implementation and scale-up.
