Book excerpt: Acquired brain injury (ABI) describes damage to the brain that occurs after birth, caused by traumatic injury such as an accident or fall, or by non-traumatic cause such as substance abuse, stroke, or disease. Today’s medical techniques are improving the survival rate for people of all ages diagnosed with ABI, and current trends in rehabilitation are supporting these individuals returning to live, attend school, and work in their communities. Yet strategies on the best way of providing community participation vary among rehabilitation experts. Because many of survivors of ABI do not and will not return to the status quo of their former lives it is important to examine what constitutes best and promisingpractices in this area. This casebook is the world’s first compilation of evidence-informed programs that foster community participation for people of all ages with brain injury. With thisreview, we elicited and carefully examined existing programmatic efforts that combine emphasis on the individual, the social, and the service systems in a way that captures community participation as a complex process of interactive change in the person-environment relationship – programs that do not divorce ABI survivors from their contexts, and where participation efforts facilitate positive change in the social and political context. We considered community-based programs to be programs where individuals and families actively participate in their own therapy (rehabilitation) and take responsibility for their own health or that of a family/community member. Each case study chapter depicts a program chosen on its extraordinary merits to provide community participation to its clients. The chapters are cowritten by the stakeholder and a researcher, giving a complete perspective of how the program was established and continues to operate, and provides evidence of excellence.

The following report serves as a resource for patients and families wanting to gain information regarding community integration outcomes after participation in post-acute rehabilitation programs. The goal of the post-acute level of medical care is to increase functionality and serve as a transition for the patient from the rehabilitation facility to life within the community. A thorough examination of community integration after participation in a post-acute rehabilitative program with the use of the Community Integration Questionnaire (CIQ) is provided. After investigation of the available literature, four articles were found to meet inclusion criteria and were included within the review. All studies included met the following criteria. Participants were ages 17 to 65 years old, had a diagnosis of moderate to severe TBI, were enrolled in post-acute rehabilitation, and were assessed with the Community Integration Questionnaire (CIQ). Overall, the available literature suggests that completion of a program within a post-acute facility does create positive outcomes for the individual with TBI; however, the outcomes are dependent on various factors regarding TBI severity, the administration of intervention, the type of intervention, time post-onset and age of participants at the time of onset. Future research is necessary to provide a more comprehensive view of post-acute rehabilitation and the outcomes that these patients may expect as they begin their road to recovery.

Book excerpt: Traumatic Brain Injury (TBI) remains a significant health condition that touches all populations and communities, with the current TBI Continuum of Care consisting of medical acute care, rehabilitation and community integration. In the past, TBI was seen as a one-time event, with the TBI survivor expected to re-integrate back into the community after a convalescence period. Now researchers understand that a TBI, whether mild or severe, can be a chronic condition, following a survivor throughout their lifetime. This concept places even more importance on the community integration sector of the TBI Continuum of Care. Unlike the medical and rehabilitation sectors, which have universally-applied standards of care, the community integration (CI) sector does not as yet have common standards by which to measure outcomes. This means organizations providing CI services can be very diverse and can run the gamut for quality of care. This dissertation focuses on the creation of a list of proposed recommendations for community integration organizations supporting TBI survivors. The research includes interviewing of CI sites throughout California, as well as interviews of individuals throughout the TBI Continuum of Care. This list of proposed recommendation focuses on potential strategies that CI organizations can adopt to improve the quality and availability of services, continuously monitor patient outcome, and enhance organizational sustainability, thereby improving the quality of life for TBI survivors and their families in California.

Book excerpt: Background: Physical activity (PA) and sport are suggested as a non-stigmatizing approaches to address long-term problems following moderate and severe traumatic brain injury (TBI) and can positively influence community integration, mood, and quality of life. However, promoting PA and sport participation for people with TBI is challenging due to long-term sequelae associated with their injuries. Consequently, PA and sport programs must be appropriately designed. This project aims to pursue the co-creation of a community-based PA and sport program for persons with TBI and stems from a pilot program that began in January 2017 in collaboration with multiple organizational partners. The program consists of three phases (i.e., learning how to train safely, independently and for a triathlon event). Four individuals with TBI from the pilot program are currently peer mentors and share their perspectives, motivate new members and assist with the development of the project as key stakeholders. To facilitate the implementation, evaluation and sustainability of the program, formal procedures and toolkits must be co-created with researchers and stakeholders to ensure the content is tailored to the program usersu2019 needs.Objectives: 1) explore the impact of the current program on mentors, participants, and administration to inform the creation process of the new program; 2) co-construct the programu2019s logic model while identifying strategies to ensure program sustainability with the community fitness center; 3) co-create a set of clearly-defined protocols for the new peer mentor program.Design: An embedded mixed-method design will be used in which supplemental qualitative data will be collected to enhance the development of the program protocol. This design incorporates participant perceptions and experiences of the program before, during and after primary measures are administered to support aspects of the overall application and evaluation of the program.Participants: A heterogeneous convenience sample of 20 community-dwelling, adult participants (4 peer mentors, 16 active participants) with severe TBI in Eastern Canada have been purposely recruited based on their suitability for the program.Methods: In line with the participatory action research approach, there will be equitable collaboration among organizational representatives (n=3), a team of multidisciplinary researchers (n=5) and community members (mentors, n=4) in every aspect of the research process. Through an iterative process, regular scheduled sessions will take place with working group teams to clearly inform a logic model describing the necessary inputs, key components, and expected outputs of the program to facilitate the development of the programu2019s protocols.Significance: This presentation will share the process of co-creating a study protocol for this innovative community based research program with multiple stakeholders. The results will further our understanding about the factors that promote PA and sport participation for adults with moderate to severe TBI and make recommendations about working with this community.

Book excerpt: This final report describes activities and accomplishments of the Building Friendships project, which developed, implemented, evaluated, and disseminated an intervention to improve the social integration of students with traumatic brain injury (tbi). The project used an ongoing, informal team approach to bring together and mobilize key people in a student's life around the goal of enhancing the student's school- and community-based social network. The project implemented intervention strategies that are both student-centered and environment-centered. These strategies included direct training of students to enhance social skills, training parents in ways to encourage skill generalization, inservice and peer training about tbi, "peer liaisons," facilitation of involvement in school organizations, training staff of community programs, and pairing community volunteers with tbi students. Students, teachers, and family members reported increases in the number of social contacts and overall time spent with nondisabled peers and the average number of reported friends. Individual sections of the report describe the project's context, goals and objectives, conceptual framework, research participants, intervention strategies, logistical problems and modifications, findings, and impact. Extensive appendices include four articles and book chapters resulting from the project, the manual for a video program, and sample newsletter articles. (Db)
