Book excerpt: This book is the compilation of papers presented at the International Symposium on in vivo Body Composition Studies, held at the University of Toronto, Ontario, Canada, June 20 - 23, 1989. The purpose of this conference was to report on advances in techniques for the in vivo measurement of body composition and to present recent data on normal body composition and changes during disease. This conference was the most recent of several meetings on body composition studies, and follows two successful such meetings, one at Brookhaven National Laboratory in 1986, and at Edinburgh in 1988. The large number of excellent research papers and posters presented at these conferences demonstrates the rapid growth of the field and the broad interest in the subject of in vivo body composition studies. The proceedings of the Brookhaven meeting "In Vivo Body Composition Studies", is published by The Institute of Physical Sciences in Medicine, London. Both the Brookhaven and the current Toronto meeting emphasized the clinical applications, together with the techniques employed. The Edinburgh meeting placed more emphasis on the methodological problems and design of instrumentation. Because of the number of papers presented at the meeting it was necessary to ask the authors from the same institution to combine their presentations into a single paper where appropriate. The editors wish to thank the authors for their cooperation and for graciously accepting the minor revisions made to each manuscript.

This book comprises 12 chapters, and begins with a focus on amino acid analysis in food and physiological samples. Succeeding chapters go on to discuss concepts and techniques on nitrogen balance; determination of the amino acid requirements of animals; and novel methods for determining protein and amino acid digestibilities in feedstuffs. Other chapters cover measurement of protein digestion in ruminants; evaluation of protein status in humans; surgical models to measure organ amino acid metabolism in vivo; and measurement of whole-body protein content in vivo. The remaining chapters discuss estimation of protein synthesis and proteolysis in vitro; isotopic estimation of protein synthesis and proteolysis in vivo; n-glycine as a tracer to study protein metabolism in vivo; and mathematical models of protein metabolism. The aim of the Conference was to define and evaluate the scientific basis for the biological monitoring of metals. The conference was co-sponsored by the World Health Organization through its International Program on Chemical Safety and received substantial encouragement and support from the Swedish Work Environmental Fund and the United States Environmental Protection Agency. This was the second conference organized jointly by the Scientific Committee on the Toxicology of Metals and The Toxicology Division of the University of Rochester. The previous joint conference was held in 1982 on the Reproductive and Developmental Toxicity of Metals. In addition, conferences have been organized by each group (see Appendices A and B). Several of these conferences are specially relevant to the topic of the current conference. These include the joint conference mentioned above and the conferences on dose-effect and dose-response relationship held in Tokyo in 1974 and on accumulation of metals held in Buenos Aires in 1972.

Book excerpt: The critically ill patient in intensive care may present with serious metabolic alterations caused directly by the illness or secondarily by complications (e.g. infections, organ failure or sepsis) developing within a few hours of hospitaliza tion or in the following days. Among the situations which maintain and further trigger rapidly evolving altered metabolism are complex hormonal reactions, particularly those of the hypothalamus-hypophysis-adrenal axis, and abnormal stimulation of the autonomic nervous system. In fact, the sympathetic nervous system is known to cause significant metabolic alterations. For example, a surgery patient afflicted by septic complications may become hypercatabolic and experience significant nitrogen loss; the altered protein metabolism may in turn heavily influence carbohydrate and lipid metabolism as well. Thus, it is apparent that for optimal care of patients with altered metabolic functions, further knowledge is necessary regarding the physiopathology of metabolism and the physiopathological mechanisms, which alter the consump tion of principal energy substrates. Many experimental and clinical studies have investigated the metabolic aspects of individual organs or organ systems. However, for a correct evaluation of such metabolic events, in addition to studying the roles of metabolic enzymes, active metabolites, and the glutathione system, it is interesting to consider the use of indirect calorimetry as a valid and important investigative technique. The critically ill patient with major alterations in nutritional status may require artificial nutritive support administered through either parenteral or enteral routes.

A primary focus of maternal-fetal medicine has been to understand the mechanism(s) of premature delivery and develop treatment modalities for improving the length of gestation. A primary focus of neonatology has been to under stand the causes of respiratory distress in the neonate. Success has resulted, not only in the lengthening of gestation, but an improved understanding of the causes and treatment of neonatal respiratory disease. With increasing success has come the necessity to under stand the metabolic principles of the parturient, the fetal/placenta unit, and the neonate. These principles are clearly very important from multiple aspects. Increased understand ing of metabolism of the pregnant woman would explain the aberrations occurring in normal and abnormal pregnancy and improve nutritional support for the parturient. A prime example of altered metabolism is the parturient with diabetes. Understanding metabolism ofthe fetal/placenta unit is necessary to increase the probability that the fetus will be born appropriate for size irrespective of the gestational age. The various compo nents of neonatal metabolism are important, not only for understanding the changes in physiology and biochemistry occurring in the developing neonate, but the principles by which nutritional support should be provided.
