Book excerpt: In recent years a great deal of effort has been put into improving risk management in departments and in 2002 this was given further impetus when a two year risk programme was launched by the Prime Minister. This report looks at the progress made since the previous NAO report in 2000 (HC 864 1999-2000, ISBN 0105569488). It is based on a survey of the 20 main Whitehall departments, focus groups of 27 departmental risk managers, comparisons with the private sector, academic research and five case studies. The general conclusion is that significant improvement has been made but more needs to be done to make effective risk management a central part of general management processes. The ability to take risks and innovate, keep projects on track and handle complex service delivery needs to be further developed.

Book excerpt: In May 2010, the Government announced the formation of the Efficiency and Reform Group within the Cabinet Office. The Group brings many of the functions of a typical corporate headquarters together in one place in the centre of government. Its priorities are improving efficiency in central government, and wider reform of the way public services are provided, in order to make the spending reductions required by the 2010 Spending Review. The Group is responsible for various new initiatives which are designed to increase efficiency, make savings and improve value for money. These include renegotiating contracts with major suppliers; implementation of a centralised procurement process; a review of major government projects; and a new Property Unit. The NAO's review details various challenges that the Group faces. It is too soon for the NAO to reach a judgment on its success in improving the value for money of government overall. The review is intended to provide an objective baseline against which progress made by the Group can be assessed by the NAO and the Public Accounts Committee.

The initiative is expected to cost around £182 million between the years 2001-2006 and targets the most deprived local authority districts in England. It seeks to ensure the representation of diverse community needs through the provision of grants to community groups involved in improving their neighbourhoods, and support for community empowerment networks that help communities influence local decision-making. The scheme has so far supported around 25,000 separate self-help and community projects, funded directly through local voluntary sector organisations. This report focuses on the effectiveness of the single Community Programme in encouraging communities to get involved in neighbourhood renewal and regeneration schemes, and seeks to identify broader lessons of relevance to community participation initiatives across the whole of government.

It pinpoints three key issues in these success stories relating to: i) the degree of engagement by senior decision-makers; ii) the level of organisational understanding of what is needed to be an 'intelligent client' (in terms of having the necessary skills to negotiate effectively with suppliers and users); and iii) the importance of realising the benefits of change by determining at the outset what the desired benefits are and how the project will be managed to ensure these benefits are optimised.

Book excerpt: There were about 12.7 million visits to accident and emergency (AandE) services in the last year, and in around a fifth of these, patients were admitted to hospital. AandE departments are only one in a variety of NHS emergency care providers, which include ambulance services, GPs, primary care trusts, out-of-hours services; NHS Direct; and open access minor injury centres. This NAO report examines the extent of progress made towards achieving the key target for maximum total time spent waiting for AandE services, as well as with the wider modernisation of emergency care. The report's conclusions include that, given the high level of demand, there has been a significant and sustained progress towards reducing waiting times in AandE departments and improving the patient's experience, largely due to improved working practices and increased local investment. However, there is further room for improvement in the case of patients with more complex needs, such as older people and those with mental health needs; and regional variations still exist with the performance of the worst performing trusts still some way behind. Further major improvements will depend on tackling the remaining bottlenecks and barriers to modernisation within wider emergency care services. The establishment of local emergency care networks is highlighted as a promising development, as a means to secure greater emergency care service integration and joint-working.

One person in three will develop cancer at some point in their life, and in the 1990s England suffered from a higher mortality rate than other European countries. However the survival rate is improving and the latest figures show that 36 per cent of men and 49 per cent of women will survive for 5 years or more. England's position in terms of the proportion of people who die from cancer is improving relative to other comparable countries. There is though a class divide, with the better off improving more than the less well off. There are variations in service but one of the factors is that people in more deprived areas are likely to have their cancer identified at a more advanced stage. The recommendations from this report cover the need to be more effective in stopping smoking, ways of identifying cancer earlier and the need for national clinical audits.

Book excerpt: In respect of its largest defence projects there are early signs that the Ministry of Defence has begun to make realistic trade-offs between cost, time, technical requirements and the amount of equipment to be purchased. Nevertheless, the continuing variances to cost and time show the MOD needs to do consistently better. This report, which gives a progress review of the 16 largest defence projects, shows that in the last year there has been a total forecast slippage of 139 months and increase in costs of £468 million. This means that, since the projects were approved, costs have increased by £6.6 billion (around 12 per cent more than the planned cost) and the projects have been delayed by 468 months, taking almost a third longer than originally expected. It would be unrealistic to expect MOD and industry to identify every risk at the start of technically challenging projects. However, the continuing problems indicate that MOD has more to learn from historic. The MOD is accepting the capability risk and some wider costs resulting from these project delays and is having to make difficult decisions about long-term capabilities. The MOD has made a significant investment in new and upgraded helicopters to address the shortfall identified in the NAO's 2004 report. The MOD has also spent £787 million on air transport and air-to-air refuelling aircraft to support current operations and address capability gaps, such as those caused by the previously reported delays to the A400M transport aircraft.
