This literature review was specifically requested by the Federal Motor Carrier Safety Administration (FMCSA) to provide information related to its Hours of Service regulations issued in January 2004. The report contains a general literature review of the health issues from 1975 to the present, and fatigue issues from January 2004 to present, associated with commercial vehicle driver hours of service. The report also contains a literature review of references that were cited in response to a related FMCSA January 2005 Notice of Proposed Rulemaking. Strictly a literature review, the report does not contain any conclusions or recommendations.

Book excerpt: There are approximately 4,000 fatalities in crashes involving trucks and buses in the United States each year. Though estimates are wide-ranging, possibly 10 to 20 percent of these crashes might have involved fatigued drivers. The stresses associated with their particular jobs (irregular schedules, etc.) and the lifestyle that many truck and bus drivers lead, puts them at substantial risk for insufficient sleep and for developing short- and long-term health problems. Commercial Motor Vehicle Driver Fatigue, Long-Term Health and Highway Safety assesses the state of knowledge about the relationship of such factors as hours of driving, hours on duty, and periods of rest to the fatigue experienced by truck and bus drivers while driving and the implications for the safe operation of their vehicles. This report evaluates the relationship of these factors to drivers' health over the longer term, and identifies improvements in data and research methods that can lead to better understanding in both areas.

Book excerpt: This report has two main objectives. The first is to present data that may be used to create a definition of short-haul trucks in computerized data files. The second is to examine the prevalence of driver fatigue as coded in crash data files and relate it to parameters that define short-haul trucking operations. Tabulations were made of the numbers of large trucks registered in the United States and their annual travel using data from the 1992 Truck Inventory and Use Survey. Truck crash statistics were derived from the 1991-1993 Trucks Involved in Fatal Accidents file and, to a lesser extent, 1995 SafetyNet data. These tabulations were cross-classified by gross vehicle weight rating (GVWR) class, area of operation and vehicle type, and crash involvement rates per truck and per mile were generated. Three possible definitions of short-haul trucks are proposed and the different definitions are compared in terms of percentage of registered trucks and miles traveled, fatal crash involvements, fatal involvement rates per truck and per mile, and prevalence of fatigue-related fatal crash involvements. The results may assist others in making decisions about hours of service regulations for the short-haul segment of the trucking industry.

Following completion of these accident investigations, the Safety Board in 1989 issued three recommendations to the U.S. Department of Transportation (DOT) addressing needed research, education, and revisions to hours-of-service regulations. Ten years have passed since these safety recommendations were issued. In the interim, the Safety Board has issued more than 70 additional recommendations to the DOT, States, industry, and industry associations to reduce the incidence of fatigue-related accidents. In response to the three 1989 recommendations, the DOT and the modal administrations have, in general, acted and responded positively to the recommendations addressing research and education; little action, however, has occurred with respect to revising the hours-of-service regulations. Nevertheless, the Safety Board believes that support has grown in recent years to make substantive changes to these regulations. This report provides an update on the activities and efforts by the DOT and the modal administrations to address operator fatigue and, consequently, the progress that has been made in the past 10 years to implement the actions called for in the three intermodal recommendations and other fatigue-related recommendations. The report also provides some background information on current hours-of-service regulations, fatigue, and the effects of fatigue on transportation safety. As a result of this safety report, the National Transportation Safety Board issued new safety recommendations to the U.S. Department of Transportation, the Federal Aviation Administration, the Federal Highway Administration, the Federal Railroad Administration, the Research and Special Programs Administration, and the United States Coast Guard.
