Book excerpt: Objectifs : Evaluer l'impact d'une intervention orale portant sur la prévention de l'endocardite infectieuse (EI) associée à la remise d'une carte personnelle, chez des patients à haut risque d'EI. Méthodes : étude interventionnelle longitudinale unicentrique, non randomisée. 90 patients inclus, 60 prospectifs (30 dans le bras EI et 30 dans le bras prothèses valvulaires) et 30 rétrospectifs avec les 2 mêmes sous populations pour comparer à une population de référence n'ayant pas reçu l'intervention. Evaluation de la compréhension et de l'application de la prévention par questionnaire rempli par l'investigatrice, par téléphone, rappel 6 à 9 mois après. Le critère de jugement est la connaissance et l'application de 2 mesures de prévention parmi les 4 définies (désinfection cutanée, hygiène dentaire, antibioprophylaxie, consulter si fièvre). Résultats : Le taux de réponse est de 90% (81/90), taux de mortalité à 20% (7/60). Une tendance à l'amélioration de la prévention de l'endocardite, 72% des patients prospectifs connaissent au moins 2 mesures de prévention versus 60% de rétrospectifs. Significativité statistique pour les bras endocardites, 65% de patients connaissant au moins 3 mesures de prévention, versus 27% chez les EI rétrospectives. Conclusion : L'intervention standardisée mise en place permet un gain dans la prévention de l'endocardite, mais des progrès sont indispensables pour diminuer la mortalité élevée (autour de 20%, stable depuis des décennies).
