L'objectif était d'analyser l'impact de l'ETP dans la vie quotidienne de diabétiques faisant la démarche de participer à des séances car proposées près de chez eux. Une étude qualitative par entretiens individuels semi-dirigés fut réalisée auprès de 12 patients diabétiques de type 2. Ils avaient participé aux réunions diététiques (RD) et/ou à l'activité physique (AP) proposées par Diabète 49 sur Baugé/Noyant (territoire rural Est du Maine-et-Loire) de Mai 2013 à Juin 2014. Ce travail objective l'impact positif de l'ETP en proximité. La facilité d'accès allégeait les contraintes de déplacement et de temps. Les patients découvraient puis poursuivaient les programmes d'ETP. La proximité favorisait les rencontres locales. Convivialité et échange entre malades étaient une source motivationnelle. Les RD apportaient notions et astuces concrètes. Les patients développaient des compétences d'autogestion, d'auto-observation, de prises de décision. L'ETP sous forme d'AP suscitait une satisfaction inattendue. Ils retrouvaient l'envie de pratiquer une AP dans une dynamique de groupe solidaire. Cependant, des freins à l'adhésion et à la participation étaient soulevés. Cette étude a exposé le ressenti des patients pour promouvoir l'ETP en proximité et non plus seulement à l'hôpital ou dans les grandes villes. En articulant les réseaux diabète aux Maisons de Santé développées en territoire rural, l'ETP devient accessible à ceux qui en sont le plus éloignés. Elle doit s'appuyer sur des généralistes sensibilisés. Les freins sont à analyser pour intégrer l'ETP durablement en médecine ambulatoire.
